Pharmacological properties
Pharmacodynamics
A topical antiseptic for application on mucous membranes. It has antimicrobial, antifungal, analgesic, coating and deodorizing effect. Antimicrobial action of the drug Maxicold® Lore is associated with inhibition of oxidative reactions of bacterial metabolism (hexetidine is a thiamine antagonist).
The drug has a wide spectrum of antibacterial and antifungal activity, in particular against Gram-positive bacteria and fungi of the genus Candida, but Maxicold® Lore may also have effect in treatment of infections caused by, for example, Pseudomonasaeruginosa or Proteus. At a concentration of 100 mg/ml, the drug suppresses most bacterial strains. The development of resistance has not been observed.
Maxicold® Lore has a mild anesthetic effect on the mucosa.
The drug has antiviral action against influenza A viruses, respiratory syncytial virus (RS-virus), herpes simplex virus type 1, affecting the respiratory tract.
Pharmacokinetics
The local action of hexatidine lasts 10-12 hours after single use. The drug is practically not absorbed from the surface of mucous membranes and has no systemic effect. After a single application of the active substance its traces are found on the gum mucosa for 65 hours. In the plaques of the teeth the active concentrations persist for 10-14 hours after application.

As an oral hygiene product to eliminate unpleasant odor (deodorizing effect).
Prevention of superinfections in collapsing tumors of the oral cavity and larynx.

Composition per 100 ml:
Active ingredient:
- hexatidine (in terms of 100% substance) – 200 mg (0.2%).
Excipients:
- sodium saccharinate dihydrate – 40 mg,
- citric acid monohydrate – 70 mg,
- glycerol (glycerin) – 17000 mg,
- ethanol (rectified ethyl alcohol) – 4000 mg,
- lactate – 10 mg.
- lauryl (macroholauryl ether) – 1500 mg,
- cineol (eucalyptol) – 20 mg,
- peppermint leaf oil – 10 mg,
- cleaned water – up to 100.0 ml.

How to take, the dosage
Topically, after meals or between meals.
The spray is used to treat the affected areas in the mouth and throat. The following steps should be followed to use the drug correctly:
- Remove the cap from the can, place the atomizer in the appropriate opening on top of the can.
- First time you use the sprayer, press briefly several times before use (to ensure even atomisation of the contents of the bottle).
- Push the desired quantity of product in the mouth by pressing the sprayer 3-4 times until it stops. Hold your breath while dosing.
- After use, remove the nebulizer, rinse it under running water and dry it in a place out of reach of children at room temperature.
- The nebulizer should be kept in its package together with the drug.
Adults and children over 3 years: Frequency of use – 2-3 times a day. If necessary, more frequent use of the drug is possible. The course of treatment is 5-7 days.
Interaction
There is currently no information about interactions with other drugs.
Special Instructions
Children may use the product at an age when they do not resist a foreign object (sprayer) in their mouth when applying the spray and are able to hold their breath when the product is injected.
The spray should not be inhaled; eye contact should be avoided. The applicator should be rinsed with warm water after application.
Please note that the spray contains 24-48 mg of ethanol in a daily dose.
In the absence of a general reaction to taking the drug, performing potentially hazardous activities that require special attention and quick reactions (including driving) is not contraindicated.
Warning! The cylinder is under pressure. Do not incinerate, puncture or open, even if the bottle is empty.
Impact on driving and operating ability
The drug has no adverse effect on driving and performing potentially hazardous activities requiring increased concentration and quick psychomotor reactions.
Contraindications
Hypersensitivity, atrophic pharyngitis, children under 3 years of age.
Side effects
Allergic reactions; burning of the oral mucosa; if accidentally swallowed – nausea; with prolonged use, impairment of the sense of taste is possible.
If any of the side effects mentioned in the instructions worsen, or if you notice other adverse reactions not mentioned in the instructions, tell your doctor.
Overdose
Hexatidine in the indicated dosage is not toxic. Ingestion of large amounts of the drug leads to vomiting, so significant absorption is not expected.
There have been no reported cases of ethanol poisoning in overdose. Acute alcohol poisoning is very unlikely, but theoretically possible if a large dose of the drug is ingested by a young child. In any cases of overdose, consult a physician immediately.
Treatment is symptomatic: taking enterosorbents (activated charcoal), gastric lavage, plenty of fluids. Gastric lavage is necessary within 2 hours after swallowing an excessive dose.
Pregnancy use
There are no reports of any adverse effects of hexatidine during pregnancy and lactation. Before prescribing the drug to pregnant or breastfeeding women, the physician should carefully weigh the benefits of treatment for the mother against the possible risk to the fetus and child, given the lack of sufficient data on penetration of the drug through the placenta and into the breast milk.
