Pharmacodynamics:
Magnesium is a vital element found in all body tissues and is necessary for normal functioning of cells, is involved in most metabolic reactions. In particular, it is involved in the regulation of nerve impulse transmission and muscle contraction.
The body receives magnesium with food. Magnesium deficiency in the body may be observed in violation of the diet (diet) or increased demand for magnesium (with increased physical and mental stress, pregnancy, use of diuretics). Pyridoxine (vitamin B6) is involved in many metabolic processes, in the regulation of nervous system metabolism. Vitamin B6 improves the absorption of magnesium from the gastrointestinal tract and its penetration into cells.
Serum magnesium content:
– 12 to 17 mg/l (0.5 – 0.7 mmol/L) indicates a moderate magnesium deficiency;
– below 12 mg/l (0.5 mmol/L) indicates a severe magnesium deficiency.
Pharmacokinetics:
Absorption of magnesium in the gastrointestinal tract is no more than 50% of the oral dose. 99% of the body’s magnesium is intracellular. Approximately 2/3 of intracellular magnesium is distributed in bone tissue, and the other 1/3 is in smooth and transverse striated muscle tissue. Magnesium is excreted mainly in the urine. At least 1/3 of the ingested dose of magnesium is excreted in the urine.
Indications
Established magnesium deficiency, isolated or associated with other deficiency conditions, accompanied by symptoms such as:
– increased irritability;
– minor sleep disturbances;
– gastrointestinal cramps or palpitations;
– increased fatigue;
– muscle aches and spasms;
– tingling sensation.
Composition
1 tablet contains: magnesium lactate dihydrate * – 470 mg; pyridoxine hydrochloride – 5 mg.
* – equivalent to the content of magnesium (Mg++) 48 mg
Excipients:
Saccharose – 115.6 mg, kaolin heavy – 40.0 mg, acacia gum – 20.0 mg, carboxypolymethylene 934 – 10.0 mg, talc (magnesium hydrosilicate) – 42.7 mg, magnesium stearate – 6.7 mg. Tablet coating: acacia gum – 3.615 mg, sucrose – 214.969 mg, titanium dioxide – 1.416 mg, talc (magnesium disilicate) – traces, carnauba wax (powder) – traces.
How to take, the dosage
Before taking the drug it is necessary to consult with the doctor.
Adults are recommended to take 6-8 tablets per day;
Children over 6 years (body weight over 20 kg) 4-6 tablets per day.
The daily dose should be divided into 2-3 doses, taken with food. Tablets should be washed down with a glass of water.
The treatment should be stopped immediately after normalization of magnesium concentration in blood.
Interaction
Contraindicated combinations:
With levodopa: Levodopa activity is inhibited by pyridoxine (unless taking this drug in combination with intake of peripheral aromatic L-amino acid decarboxylase inhibitors). Any amount of pyridoxine should be avoided unless levodopa is taken in combination with peripheral aromatic L-amino acid decarboxylase inhibitors.
Unrecommended combinations:
Concurrent use of drugs containing phosphates or calcium salts may impair intestinal magnesium absorption.
Combinations to consider:
When prescribing oral tetracyclines, an interval of at least three hours between oral tetracycline and Magnesium B6 should be observed, because magnesium preparations reduce the absorption of tetracyclines.
Special Instructions
Information for diabetics: coated tablets contain sucrose as an excipient.
In case of severe magnesium deficiency or malabsorption syndrome, treatment begins with intravenous administration of magnesium preparations.
In cases of concomitant calcium deficiency it is recommended to eliminate magnesium deficiency before starting to take calcium preparations or food supplements containing calcium.
Frequent use of laxatives, alcohol, strenuous physical and mental stress increases the need for magnesium, which can lead to magnesium deficiency in the body.
If pyridoxine is used in high doses (over 200 mg per day) for a long time (for several months or in some cases years) sensory axonal neuropathy may develop with such symptoms as numbness, proprioceptive sensitivity disorders, tremors in the distal limbs and gradually developing sensory ataxia (movement coordination disorders). These disorders are usually reversible and disappear after discontinuation of vitamin B6.
The drug in tablet form is intended only for adults and children over 6 years. For younger children (over 1 year old) the drug in the form of oral solution is recommended.
Effect on the ability to drive vehicles and engage in other potentially dangerous activities:
No effect. There are no special recommendations.

Contraindications
- High sensitivity to the ingredients of the drug;
- Severe renal insufficiency (creatinine clearance less than 30 ml/minute);
- Children under 6 years of age.
- Children under 6 years of age;
- Fructose intolerance, glucose-galactose malabsorption syndrome, sucrose-isomaltase deficiency;
- Simultaneous use of levodopa (see “Interaction with other drugs”).
Caution: In moderate renal failure, because of the risk of hypermagnesemia.
Overdose
Symptoms: with normal renal function, magnesium overdose when ingested does not usually cause toxic reactions. However, in the case of renal failure, magnesium poisoning may develop. Overdose symptoms, intensity of which depends on magnesium concentration in blood: decrease of blood pressure, nausea, vomiting, central nervous system depression, decreased reflexes, electrocardiogram changes, respiratory depression, coma, cardiac arrest and respiratory paralysis, anuric syndrome.
Treatment: rehydration, forced diuresis. Hemodialysis or peritoneal dialysis is necessary in renal failure.
Pregnancy use
Pregnancy: Clinical experience of using the drug in a sufficient number of pregnant women has not revealed any adverse effect on fetal malformations or fetotoxic effect. The preparation Magnes B6® may be used during pregnancy only if necessary and on the physician’s recommendation.
Breast-feeding period: Magnesium penetrates into the breast milk. Avoid using the drug during lactation and breast-feeding.
