Loperamide, by binding to opioid receptors of the intestinal wall (stimulation of choline and adrenergic neurons via guanine nucleotides), reduces intestinal smooth muscle tone and motility (due to inhibition of acetylcholine and PG release).
Indications
Diarrhea of different genesis (acute and chronic), stool regulation in patients with ileostomies.

How to take, the dosage
Ingestion.
Acute diarrhea. Adults: initial dose is 4 mg, then 2 mg after each act of defecation with liquid stools. Children over 5 years of age: initial dose is 2 mg, then 2 mg after each act of defecation with liquid stools.
Chronic diarrhea. Adults: initial dose is 4 mg, then 2 mg 1-6 times a day. Children over 5 years of age: 2 mg 1-5 times daily.
Interaction
Opioid analgesics increase the risk of severe constipation.
Special Instructions
If there is no effect after 2 days of using Loperamide, a doctor should be consulted.
If constipation or bloating develop during treatment, loperamide should be discontinued. In patients with impaired liver function, close monitoring of signs of toxic CNS damage is necessary.
The loss of fluids and electrolytes must be replenished during treatment of diarrhea.
During treatment, caution should be exercised while driving vehicles and engaging in other potentially hazardous activities that require increased concentration and quick psychomotor reactions.
Contraindications
Side effects
Allergic reactions (skin rash), drowsiness or insomnia, dizziness, hypovolemia, electrolyte disturbances; dry oral mucosa, colic, gastralgia, pain or discomfort in the stomach, nausea, vomiting, flatulence.
Rarely – urinary retention, extremely rare – intestinal obstruction.
Overdose
Symptoms: central nervous system depression (stupor, coordination disorders, drowsiness, miosis, muscle hypertonia, respiratory depression), intestinal obstruction.
Treatment: the antidote is naloxone; given that loperamide has a longer duration of action than naloxone, the latter may be reintroduced.
Symptomatic treatment: activated charcoal, gastric lavage, artificial lung ventilation.
Medical observation for at least 48 hours is necessary.
