Lincomycin hydrochloride, solution for intravenous and intramuscular administration, is indicated for treatment of severe infections caused by susceptible strains of microorganisms (see section “Pharmacological properties”). Its use should be reserved for patients in whom penicillin cannot be used or who are allergic to penicillin.
Because of the risk of pseudomembranous colitis associated with lincomycin, the severity of the infection and the possibility of using less toxic drugs (e.g., erythromycin) should be considered when prescribing lincomycin.
Surgical procedures may be performed in conjunction with antimicrobial therapy.
The simultaneous use of lincomycin with other antimicrobial drugs is possible if indicated.
Lincomycin should not be used in mild bacterial infections or viral diseases.
In order to reduce the development of drug-resistant bacteria and to maintain the effectiveness of lincomycin and other antibacterial agents, lincomycin should only be used to treat or prevent infections with a proven or suspected susceptible microorganism. If data on the type and sensitivity of the infectious agent are available, they are used to select or modify antibiotic therapy. In the absence of such data, local knowledge of the epidemiology and sensitivity of the microorganisms may be used for empirical treatment.

How to take, the dosage
Adults:
Intramuscularly, 600 mg (2 ml) every 24 hours. For more severe infections, 600 mg (2 ml) every 12 hours or more often.
Intravenously, from 600 mg (2 ml) to 1,000 mg every 8-12 hours. Doses may be increased if the course is more severe. In life-threatening situations, it is possible to administer up to 8 grams per day.
Children over 1 month of age:
Intramuscularly in a dose of 10 mg/kg body weight every 24 hours. In more severe course – 10 mg/kg body weight every 12 hours or more often.
Intravenously, 10 to 20 mg/kg/day, depending on the severity of the infection. Can be used in divided doses as described above for adults.
Note: Severe cardiopulmonary reactions have been observed when lincomycin hydrochloride is given at higher doses and rates of administration.
Before intravenous administration a dilution of the antibiotic is necessary (1 g lincomycin hydrochloride is diluted in at least 100 ml of an appropriate solution (see section “Interaction with other medicinal products”). Intravenous lincomycin hydrochloride is administered only by drip, the duration of infusion should be at least 1 hour.

Physically compatible: 5% dextrose solution, 10% dextrose solution, 5% dextrose and 0.9% sodium chloride, 10% dextrose and 0.9% sodium chloride, Ringer’s solution, infusion solutions with B vitamins, Infusion solutions with group B vitamins and ascorbic acid, penicillin, cephalothin, tetracycline, cephaloridin, colistimethate sodium, ampicillin, methicillin, chloramphenicol, polymyxine sulfate. Physically incompatible: novobiocin, kanamycin.
In studies, antagonism between lincomycin and erythromycin has been observed. Because of possible clinical relevance, these drugs should not be used simultaneously.
Antidiarrheal drugs reduce the effect of lincomycin (interval between their use should be at least 4 hours). Lincomycin has the ability to block neuromuscular conduction, so care should be taken when using it in conjunction with blockers of neuromuscular conduction. It increases the effect of drugs for inhalation anesthesia, muscle relaxants and opioid analgesics, increasing the risk of neuromuscular block and respiratory arrest.
Please note that compatibility and incompatibility determinations have been established based on physical observations only, not chemical analyses. Adequate clinical evaluation of the safety and efficacy of these combinations has not been performed.

Symptoms: increased severity of side effects, diarrhea, colitis.
Treatment: symptomatic therapy. Specific antidote to lincomycin is unknown. Hemodialysis and peritoneal dialysis are ineffective.
