Carbocysteine, being a mucolytic, acts on the gel phase of the endobronchial secretion, breaking disulfide bridges of glycoproteins and thus reducing viscosity and increasing elasticity of the secretion.
Carbocysteine activates sialic acid transferase – an enzyme of bocalytic cells of the bronchial mucosa, normalizes the quantitative ratio of acidic and neutral sialomucins, promotes mucous membrane regeneration, normalizes its structure, stimulates the activity of ciliated epithelium, restores the secretion of immunologically active IgA (specific protection), improves mucociliary clearance.
Pharmacokinetics
Carbocysteine is rapidly absorbed after oral administration, reaching maximum serum levels after 2 hours. Bioavailability is low (less than 10% of the dose taken) as a result of possible rapid passage through the liver. The elimination half-life is about 2 hours. It is excreted mainly by the kidneys together with the metabolites.
Indications
- Acute and chronic bronchopulmonary diseases accompanied by the formation of viscous, difficult to separate sputum (tracheitis, bronchitis, tracheobronchitis, asthma, bronchiectatic disease) and mucus (inflammatory diseases of the middle ear, nose and its accessory sinuses – rhinitis, otitis media, sinusitis).
- Preparing the patient for bronchoscopy or bronchography.

Excipients:
sucrose – 40 g;
methylparahydroxybenzoate – 0.15 g;
caramel dye, powder (E150) – 0.0344 g;
cinnamon oil – 0.002 g;
aromatic elixir (ethyl alcohol – 55.3%, rum – 42.7%,
rum aromatic additive – 2.0%) – 2 ml;
sodium hydroxide – q.s. to pH 6.1-6.3;
Purified water – up to 100 ml.
How to take, the dosage
Orally.
One measuring cup of syrup (15 ml) contains 750 mg of carbocysteine. Take 3 times a day, one measuring cup (15 ml), preferably separately from meals. The duration of treatment should not exceed 8 days.
Interaction
Carbocysteine increases the effectiveness of GCS and antibacterial therapy of inflammatory diseases of the upper and lower respiratory tracts, potentiates the bronchodilator effect of theophylline. The activity of carbocysteine is weakened by anti-cough and atropine-like agents.
Because of the alcohol content, Libexin Muco® (for adults) is prescribed with caution with the following drugs: drugs that cause adverse reactions (fever, redness, vomiting, tachycardia) when taken simultaneously with alcoholic beverages – disulfiram, cefamandol, cefoperazone, latamoxef (cephalosporin antibiotics), chloramphenicol (phenicol antibiotic), chlorampropamide, glibenclamide, glipizide, tolbutamide (sulfamide antidiabetic drugs), griseofulvin (antifungal drug), nitro-5-imidazoles (metronidazole, ornidazole, secnidazole, tinidazole), ketoconazole, procarbazine (cytostatic) drugs that depress the CNS.
Special Instructions
The alcohol content of Libexin Muco® (for adults) is 1.64 vol.%, i.e. 0.2 g of alcohol per measuring cup. It may be dangerous for people with liver diseases, alcoholism, epilepsy, brain injuries or diseases, pregnant women.
When prescribing Libexin Muco® to patients with diabetes or patients on a low-carbohydrate diet, take into account the sucrose content – 6 g per measuring cup of syrup for adults or 3.5 g per measuring spoonful of syrup for children.
If patients are on a no-salt or low-salt diet, the sodium content (97 mg) per scoop should be taken into account.
Impact on the ability to drive a car or perform work requiring increased speed of physical and mental reactions. Impairment of psychomotor reaction speed associated with the presence of alcohol in the drug may be dangerous when driving or operating machinery.

Contraindications
- High sensitivity to carbocysteine or other drug components.
- Gastric and duodenal ulcer in the acute phase.
- Cronic glomerulonephritis (acute phase), cystitis.
- Pregnancy.
- Children under 15 years.
With caution
Gastric and duodenal ulcer in the anamnesis, lactation.
Side effects
Gastrointestinal tract: sometimes nausea, vomiting, diarrhea, epigastric pain, gastrointestinal bleeding.
Allergic reactions: itching, urticaria, exanthema, angioedema.
Other: dizziness, weakness, malaise.
Overdose
Symptoms: gastralgia, nausea, diarrhea.
Treatment: symptomatic.
