Pharmacological action – replenishing vitamin E deficiency.
Pharmacodynamics
Biologically active tocopherols protect cellular respiration intermediates from oxidation of the functional groups of their molecules. Active tocopherols are able to oxidize into semiquinones, which are reduced back to tocopherols in the body.
If sufficient tocopherols are present in the body, they protect unstable mediators and metabolites of cellular respiration from oxidation.
In redox processes at the level of cellular metabolism under pathological conditions, they play a buffering role.
Vitamin E (α-tocopherol), the most effective of the tocopherols, in the human body acts as a cellular antioxidant and a substance that binds free oxygen radicals.
Pharmacokinetics
Absorption from the duodenum (the presence of bile acids salts, fats, normal functioning of the pancreas is necessary) is 20-40%. With increasing dose the degree of absorption decreases. Tmax – 4 hours. It is deposited in all organs and tissues, especially in adipose tissue.
It penetrates through the placenta in insufficient quantities: 20-30% of the concentration in the blood of the mother penetrates into the fetus. It penetrates into breast milk.
Excretion mainly through the gastrointestinal tract, with bile – more than 90%, less than 6% is excreted by the kidneys as glucuronides and other metabolites.

Interaction
Enhances the effect of GCS, NSAIDs, cardiac glycosides.
It increases the effectiveness of antiepileptic drugs in patients with epilepsy (who have increased levels of lipid peroxidation products in the blood).
High doses of iron increase oxidative processes in the body, which increases the need for vitamin E.
The simultaneous use of vitamin E in a dose greater than 400 ME/d with anticoagulants (coumarin and indandion derivatives) increases the risk of hypoprothrombinemia and bleeding.
Vitamin E increases the effect of anticoagulants if doses exceed 400 ME/day.
The simultaneous use of other vitamin complexes containing vitamin E is not recommended to avoid overdose.
Prescribing vitamin E in high doses may cause vitamin A deficiency in the body. Colestiramine, colestipol, and mineral oils reduce vitamin E absorption.
Directions for use
Ingestion. The drug is intended to be taken only by adult patients.
Treatment of vitamin E hypovitaminosis – 100 mg capsules once a day until the symptoms of hypovitaminosis disappear.
Hypovitaminosis of vitamin E accompanied with degenerative and proliferative changes in the musculoskeletal system, in particular muscular dystrophy, amyotrophic lateral sclerosis (in combination therapy) – 100 mg capsules 1 time daily for 1-2 months.
Hypovitaminosis E with chronic hepatitis (in complex therapy) – 100 mg capsules once a day.
Hypovitaminosis E in conditions of convalescence after severe diseases and injuries and surgeries – 100 mg capsules once a day for 1 to 2 weeks.
Hypovitaminosis E with increased physical activity – 100 mg capsules once a day during periods of intense physical activity.
Hypovitaminosis E with menstrual disorders (as part of complex therapy in the hormonal treatment of menstrual disorders) – 300-400 mg cap. every day sequentially, starting from the 17th day of the cycle till the last day of the cycle (the day before the first day of menstruation).
Special Instructions
In congenital bullous epidermolysis, white hair may begin to grow in areas affected by alopecia.
Deficiency of vitamin K in the body, causing hypoprothrombinemia, may be increased with high doses of vitamin E (over 400 IU/day). Vitamin E increases the effect of anticoagulants if doses exceed 400 ME/day. In order to avoid overdose, it is not recommended to take other vitamin complexes containing vitamin E at the same time.
Impact on the ability to drive vehicles or mechanisms. Administration of Vitamin E Zentiva, Capsules does not affect the person’s ability to drive vehicles or operate machinery.
Contraindications
With caution: In patients with severe cardiosclerosis, myocardial infarction, increased risk of thromboembolism; hypothrombinemia caused by vitamin K deficiency may worsen when vitamin E is used at a dose greater than 400 ME (1 mg – 1.21 ME).
Overdose
Symptoms: high doses of vitamin E (400-800 mg/day for a long time) can cause visual disturbances, diarrhea, dizziness, headache, nausea, severe fatigue, fainting, and white hair growth on areas of alopecia in bullous epidermolysis. Very high doses (exceeding 800 mg for a long time) may cause bleeding in patients with vitamin K deficiency; they may disrupt thyroid hormone metabolism and increase the risk of thrombophlebitis and thromboembolism in sensitive patients.
Treatment: symptomatic, drug withdrawal, prescription of GCS.
