Inflammatory, anti-allergic, antibacterial, antifungal agent.
Clotrimazole has antifungal effect by disrupting the synthesis of ergosterol, which is a part of the cell membrane of fungi. It is active against; Trichophyton rubrum, Trichophyton mentagrophytes, Epidermophyton floccosum, Microsporum canis, Candida albicans, Malassezia furfur (Pityrosporum orbiculare).
Beclomethasone has anti-inflammatory, anti-allergic, antiexsudative and antipruritic effects. It inhibits leukocyte accumulation and release of lysosomal enzymes and inflammatory mediators, inhibits phagocytosis, reduces vascular and tissue permeability and prevents formation of inflammatory edema.
Gentamicin is a broad spectrum antibiotic of aminoglycoside group with bactericidal effect against Gram-negative bacteria – Pseudomonas aeruginosa, Enterobacter aerogenes, Escherichia coli, Proteus vulgaris, Klebsiella pneumoniae, Gram-positive bacteria: Streptococcus spp, Staphylococcus aureus.
Pharmacokinetics
Pharmacokinetic studies of the drug have not been conducted.
Indications
- dermatoses complicated by secondary infection
- foot epidermophytosis
- dermatomycoses (including those localized in the groin area) caused by pathogens sensitive to the drug.
Composition
1 gram of cream contains:
active ingredients:
- clotrimazole – 10.0 mg,
- beklometasone dipropionate – 0.25 mg,
- gentamicin sulfate, which is equivalent to gentamicin-1 mg
excipients: Propylene glycol, cetomacrogol, petroleum jelly, liquid paraffin, benzyl alcohol, methyl parahydroxybenzoate (methylparaben), propyl parahydroxybenzoate (propylparaben), butylhydroxytoluene, sodium dihydrophosphate monohydrate, sodium hydrophosphate, purified water.
How to take, the dosage
Externally. The cream is applied thinly to the affected areas of the skin twice a day (morning and evening). The duration of treatment depends on the effectiveness and tolerability of the therapy and is usually 2-4 weeks (no more).
Children (7-16 years) drug is prescribed only with the prescription and under medical supervision. Side effects Burning sensation, hyperemia, erythema, flaking, edema, dryness, maceration and atrophy of the skin; stricture, hypertrichosis, folliculitis, sweat, urticaria, steroid acne, perioral dermatitis, allergic contact dermatitis, telangiectasia, hypopigmentation, pyoderma, furunculosis.
Long-term use may cause systemic side effects including inhibition of adrenal cortical function.
Interaction
Interaction of the drug with other medicines has not been revealed.
Special Instructions
The drug is not used in ophthalmology. Do not apply to the skin in the eye area. In order to increase the effectiveness of therapy it is possible to use occlusive dressings.
It should be noted that transdermal absorption of beclomethasone and gentamicin increases, which leads to increased risk of systemic side effects.
If resistant bacterial or fungal microflora is manifested, the drug should be discontinued and appropriate therapy should be administered. Cross-resistance with aminoglycoside antibiotics may develop.
Contraindications
Skin tuberculosis, dermal manifestations of syphilis, varicella, herpes simplex, skin reactions after vaccination, hypersensitivity to the drug components, children under 7 years of age, pregnancy, lactation, open wounds.
Overdose
If recommended doses are exceeded, symptoms of hypercorticism may develop: weight gain, edema formation, hyperthesia, glucosuria, hypokalemia. Treatment: symptomatic therapy, correction of electrolyte balance if necessary. Gradual withdrawal of the drug is recommended.
