Iodine is one of the vital trace elements. Without iodine normal functioning of the thyroid gland is impossible, because it is an integral part of thyroxine and triiodothyronine. Thyroid hormones are involved in the development of all organs and systems, in the regulation of metabolic processes in the body: protein, fat, carbohydrate and energy, they also regulate the activity of the brain, nervous system and CSS, mammary glands, growth and development of children, the formation of its intellectual abilities. Iodine deficiency is especially dangerous for children, adolescents, pregnant and lactating women.

Iodobalance®, a source of iodine, fills its shortage in the body, prevents the development of iodine deficiency diseases, prevents the development of goitre associated with iodine deficiency in food; normalizes the size of the thyroid gland in newborns, children, adolescents and adults.

Pharmacokinetics

When taken orally, the drug is almost completely absorbed in the small intestine. The average Vd for healthy people is approximately 23 liters (38% of body weight). Plasma iodine concentration in normal blood is 0.001-0.005 mcg/ml. It accumulates in the thyroid gland, salivary glands, mammary glands and gastric tissues.

The concentration in saliva, gastric juice and breast milk is about 30 times higher than in blood plasma. Excreted in the urine, the concentration of iodine in the urine relative to creatinine (µg/g) is an indicator of its intake.

Indications

• Prevention of iodine deficiency diseases, including. endemic goiter (especially in pregnant and nursing women);
• prevention of goiter recurrence after surgical removal or after goiter treatment with thyroid hormones;
• treatment of diffuse euthyroid goiter in newborns, children, teenagers and young adult patients.

How to take, the dosage

• For the prevention of endemic goiter
  • Infants and children: 50-100 micrograms of iodine / day.
  • Adolescents and adults: 100-200 mcg iodine/day.
  • Pregnancy and lactation (breastfeeding): 200 mcg of iodine/day.

• To prevent relapse of the goiter after its surgical removal or upon completion of treatment with thyroid hormone drugs

  100-200 mcg of iodine daily.

• In the treatment of euthyroid goiter

  Newborns, children, adolescents, adult patients of young age are prescribed 100-200 mcg of iodine/day.

  The daily dose should be taken in 1 dose, after a meal, with plenty of fluids.

• Preventive use of the drug

  Held for several months or years, often over a lifetime.

• Treatment of goiter
  • In newborns: 2-4 weeks is sufficient in most cases.
  • In children, adolescents and adults: usually 6-12 months or more.

Interaction

The effectiveness of treatment with antithyroid drugs with concomitant administration of iodine is reduced.

Potassium perchlorate suppresses iodine absorption by the thyroid gland.

The administration of high doses of iodine and concomitant administration of potassium-saving diuretics may lead to hyperkalemia.

The concomitant administration of high doses of iodine with lithium preparations promotes goiter and hypothyroidism.

Iodine uptake by the thyroid gland and its metabolism is stimulated by TTG.

Special Instructions

It should be taken into account that during therapy with the drug, patients with renal insufficiency may develop hyperkalemia.

Hyperthyroidism or nodular toxic goiter as well as a history of these diseases should be excluded before initiating therapy.

In the presence of predisposition to autoimmune thyroid disease, antibodies to thyroperoxidase may be formed.

The saturation of the thyroid gland with iodine may prevent the accumulation of radioactive iodine used for therapeutic or diagnostic purposes. Because of this, it is not recommended that the drug be taken before performing radioiodine actions.

Contraindications

• • Expressed thyrotoxicosis.
  • Hidden thyrotoxicosis (when used in doses >150 mcg/day)
  • During dermatitis herpetiformis.
  • Toxic adenoma, nodular goiter when used in doses of >300 mcg/day (except preoperative therapy for thyroid blockade).
  • Iodine hypersensitivity.
  • Hypothyroidism (except when the development of the latter is caused by severe iodine deficiency).

With caution:

• In therapy with radioactive iodine.
• In the presence or suspected presence of thyroid cancer.

Side effects

In prophylactic use of Iodobalance, as well as in therapeutic use in infants, children and adolescents, side effects are usually not observed.

Overdose

Symptoms: Iodine-induced hyperthyroidism may develop in patients with foci of functional autonomy in the thyroid gland when the drug is prescribed in a dose greater than 150 mcg/day.

In therapy with iodine preparations at very high doses (>1000 mcg/day) iodine-induced goiter and hypothyroidism may develop in individual cases.

Chronic overdose may lead to the development of iodism – metallic taste in the mouth, swelling and inflammation of mucous membranes (rhinitis, conjunctivitis, gastroenteritis, bronchitis), acne, dermatitis, salivary gland swelling, temperature increase, irritability.

Treatment: in chronic overdose it is recommended to stop using the drug. In the development of iodine-induced hyperthyroidism it is recommended to stop using the drug and prescribe therapy with antithyroid drugs. In particularly severe cases, intensive therapy, plasmapheresis or thyroidectomy should be carried out. In the development of hypothyroidism it is recommended to stop using the drug and prescribe therapy with iodine containing thyroid hormones.
