Hydrocortisone is a synthetic glucocorticosteroid drug for external use. It has anti-inflammatory, anti-edematous, antipruritic effects.
Inhibits release of cytokines (interleukins and interferon) from lymphocytes and macrophages, inhibits release of inflammatory mediators by eosinophils, inhibits metabolism of arachidonic acid and synthesis of prostaglandins.
Stimulating glucocorticosteroid receptors, induces formation of lipocortin. It reduces inflammatory cell infiltrates, decreases migration of leukocytes, including lymphocytes to the inflammatory area. When administered in recommended doses it does not cause systemic side effects.
Pharmacokinetics
After application, it accumulates in the epidermis (mainly in the granular layer). It is metabolized directly in the epidermis and later in the liver. It is excreted by the kidneys and intestines.

Inflammatory and allergic skin diseases of non-microbial etiology (including those accompanied by itching):

How to take, the dosage
Topically. The ointment is applied in a thin layer on the affected areas of the skin 2-3 times a day.
The course of treatment depends on the nature of the disease and effectiveness of therapy; as a rule, it is 6-14 days; if the disease persists, it can be prolonged up to 20 days.
On limited foci, occlusive dressings can be used to enhance the effect.
When using the ointment in children 2 years and older, the total duration of treatment should be limited and activities that lead to increased resorption of steroid absorption (warming, fixation and occlusive dressings) should be excluded.
Interaction
Barbiturates, antiepileptics and antihistamines reduce efficacy.
Paracetamol promotes hepatotoxicity.
Decreases the activity of salicylates, hypoglycemic agents, increases the activity of anticoagulants.
Heart glycosides and non-potassium-saving diuretics increase hypokalemia, anabolic steroids increase tissue hydrophilicity.
When combined with amphotericin B, development of dilated myocardial damage and heart failure is possible.
Special Instructions
In cases of use in children, on the face or under occlusive dressings, the duration of treatment should be shortened.
In children, suppression of adrenal cortical function may develop more rapidly.
In addition, decreased excretion of growth hormone may be observed. Body weight, growth, and plasma cortisol levels should be monitored if the drug is used for an extended period of time.
Contact of the ointment into the eyes should be avoided. Children under 12 years of age are prescribed the drug only under strict medical supervision. If after 7 days of use no improvement or deterioration is observed, or if symptoms return within a few days after discontinuation of the drug, the use of the drug should be stopped and a physician should be consulted. If prolonged treatment and/or application on large surfaces, it is recommended to prescribe a diet with sodium restriction and increased K+ content; sufficient protein should be administered.
The BP, blood glucose, blood clotting, diuresis, the patient’s body weight, and plasma cortisol levels should be monitored.
Contraindications
With caution
Diabetes mellitus, tuberculosis (systemic lesion).
Side effects
Skin atrophy, hypertrichosis, secondary infection, swelling, itching, hyperemia, allergic reactions.
When prolonged use of high doses – side effects typical for systemic use of glucocorticoids.
Overdose
Long-term use of the drug in high doses may cause symptoms of hypercorticism.
