Pharmacodynamics

The mucolytic and expectorant action of Fluditek is due to the activation of sialic transferase – an enzyme of the mucous membrane of the bronchi. Normalizes the quantitative ratio of acidic and neutral sialomucin bronchial secretion, restores viscosity and elasticity of mucus. Contributes to the regeneration of the mucous membrane, normalizes its structure, activates the ciliated epithelium. Restores secretion of immunologically active IgA (specific protection) and the number of sulfhydryl groups of mucus components (nonspecific protection), improves mucociliary clearance.

Pharmacokinetics

After oral administration Cmax in blood serum and in respiratory mucosa is reached after 2-3 hours and persists in mucosa for 8 hours. It is excreted mainly by the kidneys, partially – unchanged, partially – as metabolites.

Indications

– acute and chronic bronchopulmonary diseases accompanied by formation of viscous, difficult to separate sputum (tracheitis, bronchitis, tracheobronchitis, bronchial asthma, bronchiectatic disease);

– Acute and chronic inflammatory diseases of the nasal cavity, nasopharynx, sinuses and middle ear, accompanied by the formation of viscous, hard-to-drain mucus (rhinitis, adenoiditis, sinusitis, otitis media);

– preparation for bronchoscopy and/or bronchography.

How to take, the dosage

For children over 5 years old the daily dose is 300 mg, i.e. 1 teaspoon (5 ml) 3 times a day.

In children from 2 to 5 years of age the daily dose is 200 mg/day, i.e. 1 teaspoon 2 times/day.

In newborns and children from 1 month to 2 years – 20-30 mg/kg/day in 1 or 2 doses, and the dose of 100 mg/day should not be exceeded.

The duration of treatment, as a rule, should not exceed 8-10 days.

Interaction

When used together with GCS a synergistic effect is noted.

It increases the effectiveness of antibiotic therapy of inflammatory diseases of the upper and lower respiratory tract.

Magnifies the bronchodilator effect of theophylline.

The activity of carbocysteine is weakened by anti-cough and atropine-like agents.

Special Instructions

With caution should be used Fluditek in wet cough.

Persons with peptic ulcer disease of the stomach and duodenum are advised to prescribe the drug with caution.

The concomitant use of Fluditec with cough suppressants and/or substances that depress bronchial secretion (including atropine-like agents) is not recommended.

The sucrose content (3.5 g per teaspoon in the syrup for children) should be considered when prescribing the drug in concomitant diabetes, as well as in patients on a low-sugar diet.

Contraindications

– gastric and duodenal ulcer in the acute phase;
– chronic glomerulonephritis (in the acute phase), cystitis;
– Children under 2 years of age (for the syrup 20 mg/ml for children) and under 15 years of age (for the syrup 50 mg/ml);
– 1st trimester of pregnancy (for the syrup 50 mg/ml);
– Hypersensitivity to carbocysteine or other components of Fluditec preparation.

The drug should be used with caution in chronic glomerulonephritis (in anamnesis), gastric and duodenal ulcer (in anamnesis).

Side effects

Gastrointestinal organs: sometimes – nausea, vomiting, diarrhea, epigastric pain, gastrointestinal bleeding.

Allergic reactions: in single cases – itching, urticaria, exanthema, angioedema.

Nervous system and sensory organs: dizziness, weakness, malaise.

Overdose

Symptoms: gastralgia, nausea, diarrhea.

Treatment: symptomatic therapy.

Pregnancy use

It is contraindicated in the first trimester of pregnancy.
