Etoxysclerol has a venosclerosing, local anesthetic effect. It damages the vascular endothelium, causing denaturation of proteins, stimulates rapid thrombus formation and its organization within 7 days.
Ethoxysclerol reduces the excitability of nerve endings and the conduction of the pain impulse, ensuring painless sclerotherapy.
Pharmacokinetics
The volume of distribution is 24.5 L, binding to plasma proteins is 64%, T1/2 is 4 hours. Total Cl – 11.7 l/h, renal – 2.43 l/h, biliary – 3.14 l/h. It does not cumulate with repeated injections.
Indications
Sclerosing varicose veins and telangiectasias, hemorrhoids;
Obliteration or parietal sclerosing of varicose veins of the esophagus and the stomach in case of acute bleeding from varicose veins of the esophagus and the condition after it, total thrombosis of the portal vein, shunt thrombosis in intrahepatic block, etc.д.

How to take, the dosage
The total dose of polidocanol in sclerosing varicose veins and hemorrhoids should not exceed 2 mg/kg body weight per day. For a patient weighing 70 kg you can thus administer up to 140 mg of polydocanol (28 ml of 0.5% etoxysclerol solution, 14 ml of 1% etoxysclerol solution, 4.6 ml of 3% etoxysclerol solution).
Sclerosing varicose veins and telangiectasias: strictly intravenously with the leg horizontal or elevated at 30-45 °, using 0.5-3% solution in ampoules.
Sclerosing subcutaneous varicose veins: 0.1-0.3 ml of 0.5% solution per injection (depending on the size of the sclerosing veins).
Sclerosing telangiectasia: 0.1-0.2 ml of 0.5% solution per injection.
Central vein sclerosing for telangiectasia: 0.1-0.2 ml of 1% solution per injection.
Sclerosing of small varicose veins: 0.1-0.3 ml of 1% solution per injection.
Sclerosing medium varicose veins: 0.5-1 ml of 2-3% solution per injection. At the first administration (especially in patients prone to hypersensitive reactions), one injection is made; at the following ones, several injections can be made, taking into account the maximum dose.
Sclerosing of hemorrhoids: use 3% solution in ampoules, the first single dose – 0.5-2 ml of 3% solution strictly under the mucous membrane above the node (in men, with a node located at 11 hours, the injection is no more than 0.5 ml), repeatedly – in 1-2 weeks, 3 ml of 3% solution.
Sclerosing of varicose veins of the esophagus and stomach. The total dose of polidocanol should not exceed 4 mg/kg of body weight. It is used 0.5-1% solution in vials, mainly paravasally (submucosal, subepithelial) in the lower third of the esophagus (intravasal and combined – para- and intravasal injections are possible). At paravasal injection causes local edema squeezing varicose veins and facilitates scarring of the esophageal wall. In case of bleeding – 5-15 ml (up to 30 ml) of 0.5% solution in 1 ml portions until bleeding stops; if there is no bleeding – paravasally up to 40 ml of 0.5% solution in 1 ml portions or intravasally – no more than 20 ml in 2 ml portions; at 5-7 day intervals the procedure is repeated 2-4 times with 1% solution.
Interaction
It enhances the arrhythmogenic effect of general anesthetics.
Special Instructions
It should not be administered intra-arterially. Small amounts of 0.5% solution are recommended for ankle and ankle applications.
In sclerosing saphenous veins and telangiectasias, the injection site is tamponed and a compression dressing immediately applied; the patient should walk for 30 minutes after the procedure.
The dressing compresses vein walls, which prevents recanalization and promotes fibrous scar formation.
In case of sclerosing telangiectasia the dressing is applied for 2-3 days, in case of significant varicose veins compression with elastic bandages or class II-III medical stockings is recommended for 4-6 weeks.

Side effects
In sclerosing of varicose veins of the extremities: hyperpigmentation of the skin at the injection site.
In sclerosing hemorrhoids: transient pain.
In sclerosing varicose veins of the esophagus and stomach: necrosis, ulceration and stenosis of the esophagus, exudation into the pleural cavity, dysphagia, fever, anaphylactic shock.
Pregnancy use
It is contraindicated in the first trimester of pregnancy and after 36 weeks; it is recommended to stop breastfeeding for 2-3 days after administration of Etoxysclerol.
