L-carnitine increases resistance of nervous tissue to damaging factors (hypoxia, trauma, intoxication, etc.), inhibits the formation of keto acids and anaerobic glycolysis, reduces the degree of lactacidosis.
L-carnitine is a natural substance related to B vitamins. It participates in metabolic processes as a carrier of fatty acids across cell membranes from the cytoplasm to the mitochondria where they are oxidized (beta-oxidation process) producing large amounts of metabolic energy (in the form of ATP).
The preparation replenishes alkaline blood reserve, restores autoregulation of cerebral hemodynamics and increases blood supply of the affected area, accelerates reparative processes in the lesion and has an anabolic effect.
Indications
– as part of the complex therapy for acute hypoxic conditions (acute cerebral hypoxia, ischemic stroke, transient ischemic attack);
– acute, subacute and recovery periods of cerebral circulatory disorders;
– discirculatory encephalopathy and various traumatic and toxic brain lesions, in the recovery period after surgical interventions;
– primary and secondary carnitine deficiency, includingÑ. In patients with chronic renal failure who are on hemodialysis;
– cardiomyopathy;
– CHD (angina pectoris, acute myocardial infarction, postinfarction states);
– hypoperfusion due to cardiogenic shock and other myocardial metabolic disorders.
Active ingredient
Composition
1 ml of the solution contains:
levocarnitine (carniphite) – 100 mg;
water for injection up to 1 ml.
How to take, the dosage
Elcar is administered intravenously by slow drip or stream (2-3 min) or intravenously. Before intravenous injection, the contents of the ampoule are dissolved in 100-200 ml of solvent (0.9% sodium chloride solution or 5% dextrose (glucose) solution).
In acute disorders of cerebral circulation 1 g/day is prescribed for 3 days, and then 0.5 g/day for 7 days. After 10-12 days repeated courses of 3-5 days are possible.
When the drug is prescribed in the subacute and recovery period, in discirculatory encephalopathy and different brain lesions, carnitine deficiency patients are administered Elkar solution at a rate of 0.5-1 g/day intravenously (drop or stream) or by injection (2-3 times a day) without dilution for 3-7 days. If necessary the second course is prescribed in 12-14 days.
Intravenous injection, slowly (2-3 minutes).) is administered when secondary carnitine deficiency in hemodialysis – 2 g once (after the procedure), in acute myocardial infarction, acute heart failure – 3-5 g/day divided into 2-3 doses during the first 2-3 days with subsequent reduction of the dose by 2 times; in cardiogenic shock – 3-5 g/day divided into 2-3 doses before the patient goes out of shock. Then switch to oral administration of Elcar.

The drug does not affect the performance of potentially dangerous activities requiring special attention and quick reactions (driving a car and other vehicles, working with moving mechanisms, work of dispatcher and operator, etc.п.).
Contraindications

Side effects
Allergic reactions, muscle weakness (in patients with uremia) are possible.
In fast administration (80 drops/min or more), pain along the vein may occur, which disappears when the rate of administration is reduced.
Overdose
Symptoms: dyspeptic disorders, myasthenic disorders (in patients with uremia).
Treatment: gastric lavage, taking activated charcoal.
