Diprosalik is an anti-inflammatory, antipruritic, anti-allergic, keratolytic, antimicrobial agent. Diprosalik is a combined preparation based on two active substances – betamethasone dipropionate and salicylic acid.
Betamethasone dipropionate is a glucocorticosteroid and has anti-inflammatory, anti-allergic and antipruritic effects.
Salicylic acid has keratolytic, as well as antimicrobial and antifungal properties when used topically.
Indications
To reduce the inflammatory manifestations of dry and hyperkeratic dermatoses sensitive to GCS therapy, including:
Active ingredient
Composition
1 g of the ointment contains:
Active substances:
betamethasone dipropionate0.64 mg;
salicylic acid30 mg;
Associates:
liquid paraffin;
Vaseline.
How to take, the dosage
Apply a thin layer twice a day, morning and night, completely covering the affected areas of skin.
In some patients, a maintenance effect can be achieved with less frequent applications.
The duration of the course of treatment is determined by its effectiveness, as well as by the patient’s tolerability and the presence and severity of side effects.
Special Instructions
In case of dryness, skin irritation or hypersensitization, the use of the drugs should be discontinued. In case of infection, appropriate therapy should be prescribed. Local administration of GCS, especially in children, may cause side effects similar to those of systemic GCS, including inhibition of adrenal cortical function.
The systemic absorption of GCS and salicylic acid when applied topically will be greater with occlusive dressings and if large body surfaces are treated. Appropriate precautions should be taken if prolonged use is anticipated (especially in children).
With long-term treatment, withdrawal should be done gradually.
The ointment and lotion are not intended for use in ophthalmology. Eye and mucosal contact should be avoided.
The lotion should not be used under occlusive dressings.
The product should not be applied to open wounds or damaged skin.
Pediatric use
The product can be used with caution in children over the age of 2 years.
Children are more at risk of hypothalamic-pituitary-adrenal depression with topical GCS than adults because of their higher body surface area to weight ratio and therefore increased absorption of the drug.
There have been reports of hypothalamic-pituitary-adrenal suppression, Cushing’s syndrome, delayed growth, delayed weight gain, and increased intracranial pressure in children treated with topical GCS.
The suppression of hypothalamic-pituitary-adrenal system function in children is determined by low plasma cortisol levels and no response to ACTH stimulation. Increased intracranial pressure is manifested by a protruding fontanelle, headaches, and bilateral edema of the optic disc.
Contraindications
Hypersensitivity to any component of the drug.
Side effects
Burning, itching, skin irritation and dryness, folliculitis, hypertrichosis, acne-like rashes, hypopigmentation, perioral dermatitis, allergic contact dermatitis; maceration and skin atrophy, secondary infection, stretch marks, sweating when using occlusive dressings.
Salicylic acid preparations may cause dermatitis.
In case of local application of GCS for a long period and/or in higher doses, especially in children, side effects typical for systemic GCS may develop (including suppression of hypothalamic-pituitary-adrenal system function, development of Cushing’s syndrome, growth retardation, lack of body weight gain and increased intracranial pressure).
Overdose
Symptoms: Overuse or prolonged use of topical GCSs can cause hypothalamic-pituitary-adrenal depression leading to secondary adrenal insufficiency and hypercorticism, including Cushing’s disease.
Continued use of high doses of topical salicylic acid preparations may cause symptoms of salicylate poisoning (early symptoms are nausea and vomiting).
Treatment: symptomatic therapy. Acute symptoms of hypercorticism are usually reversible. If necessary, perform correction of electrolyte imbalance.
In cases of salicylic acid poisoning, measures should be taken to quickly eliminate salicylic acid from the body, such as oral administration of sodium bicarbonate (to alkalize the urine and force diuresis).
Pregnancy use
Because the safety of topical antagonists in pregnancy has not been established, the use of this class of drugs is justified only if the potential benefit to the mother outweighs the potential risk to the fetus.
The drugs in this class should not be used in high doses or for prolonged periods of time during pregnancy.
As it is not yet clear whether the level of systemic absorption of GCS is sufficient to produce detectable concentrations in breast milk, either breastfeeding or use of the drug should be stopped for the duration of treatment, considering the mother’s need.
