Diara is an anti-diarrheal agent.
Loperamide, by binding to opioid receptors of the intestinal wall (modulation of choline and adrenergic neurons through guanine nucleotides), reduces intestinal smooth muscle tone and motility (by inhibiting the release of acetylcholine and prostaglandins).
Slows down peristalsis and increases the time of passage of intestinal contents.
It increases tonicity of anal sphincter and helps in retention of feces and in reduction of urges to defecation. The action comes on quickly and lasts for 4-6 hours.
