Pharmacodynamics

Synthetic analog of gamma-butyrobetaine, inhibits gamma-butyrobetaine hydroxylase, reduces carnitine synthesis and transport of long-chain fatty acids through cell membranes, prevents accumulation of activated forms of unoxidized fatty acids – acylcarnitine and acylcoenzyme A derivatives in cells.

As a result of a decrease in the concentration of carnitine increased synthesis of gamma-butyrobetaine, which has vasodilatory properties.

Under ischemic conditions meldonium restores the balance of oxygen delivery and consumption in cells, prevents the violation of adenosine triphosphate (ATP) transport: at the same time it activates glycolysis, which proceeds without additional oxygen consumption.

The mechanism of action determines the variety of pharmacological effects of meldonium: increased efficiency, reduction of symptoms of mental and physical stress, immune activation, cardioprotective effect.

In acute ischemic myocardial injury it slows the formation of necrotic zone and reduces the rehabilitation period.

In cases of cardiac insufficiency the drug increases myocardial contractility and tolerance to physical activity: reduces the frequency of angina attacks.

In acute and chronic ischemic disorders of cerebral circulation improves blood circulation in the focus of ischemia, redistributes blood in favor of the ischemic area.

A characteristic tonic effect on the central nervous system, elimination of functional disorders of the autonomic nervous system in withdrawal syndrome in patients with chronic alcoholism.

Pharmacokinetics

After oral administration the drug is quickly absorbed from the gastro-intestinal tract with the bioavailability of 78 %.

Maximum concentration in blood plasma is reached after 1-2 hours.

Metabolized in the body with the formation of two major metabolites, which are excreted by the kidneys.

Half-life period depends on the dose and is 3-6 hours.

Indications

For oral administration or intravenous injection: in complex therapy of CHD (angina pectoris, myocardial infarction), chronic heart failure, dyshormonal cardiomyopathy; in complex therapy of acute and chronic cerebrovascular disorders (strokes and cerebrovascular failure); reduced work capacity, physical overexertion (incl. athletes).

In order to accelerate rehabilitation; withdrawal syndrome in chronic alcoholism (in combination with specific alcohol therapy).

For parabulbar injection: acute circulatory disorders in the retina, hemophthalmia and retinal hemorrhages of various etiology, central retinal vein thrombosis and its branches, retinopathies of various etiology (including diabetic and hypertensive) – only for parabulbar injection.

Composition

The contents of the capsules is a crystalline powder of white or almost white color with a faint odor;

Powder is hygroscopic, allowed to clump.

Meldonium dihydrate 250 mg

Excipients:

Potato starch – 13.3 mg,

Colloidal silica (aerosil) – 5.7 mg,

Calcium stearate – 2.7 mg.

How to take, the dosage

Interaction

May be combined with antianginal drugs, anticoagulants, antiaggregants, antiarrhythmic drugs and diuretics and bronchodilators.

It enhances the effect of coronadilators, some hypotensive drugs, cardiac glycosides.

When combined with nitroglycerin, nifedipine, alpha-adrenoblockers. other hypotensive drugs and agents and peripheral vasodilators, caution should be exercised due to possible development of moderate tachycardia and arterial hypotension.

Special Instructions

Caution should be exercised when treating patients with chronic liver and kidney disease, especially with long-term use of the drug.

There are insufficient data on use in children.

Influence on driving and operating ability

There are no data on adverse effects of Cardionat® on driving and operating ability.

Contraindications

Hypersensitivity to the main and/or auxiliary components of the drug; increased intracranial pressure (with venous outflow disorders and intracranial tumors); pregnancy; breastfeeding period; age under 18 years (efficacy and safety not established).

With caution

Chronic liver disease and/or kidney disease (due to insufficient data).

Side effects

Overdose

Symptoms: decreased blood pressure, accompanied by headache, tachycardia, dizziness, general weakness.

The treatment is symptomatic.

Pregnancy use

In pregnancy and during lactation (breastfeeding) the use of the drug is contraindicated.
