Pharmacodynamics
Synthetic analog of gamma-butyrobetaine, inhibits gamma-butyrobetaine hydroxylase, reduces carnitine synthesis and transport of long-chain fatty acids through cell membranes, prevents accumulation of activated forms of unoxidized fatty acids – acylcarnitine and acylcoenzyme A derivatives in cells.
As a result of a decrease in the concentration of carnitine increased synthesis of gamma-butyrobetaine, which has vasodilatory properties.
Under ischemic conditions meldonium restores the balance of oxygen delivery and consumption in cells, prevents the violation of adenosine triphosphate (ATP) transport: at the same time it activates glycolysis, which proceeds without additional oxygen consumption.
The mechanism of action determines the variety of pharmacological effects of meldonium: increased efficiency, reduction of symptoms of mental and physical stress, immune activation, cardioprotective effect.
In acute ischemic myocardial injury it slows the formation of necrotic zone and reduces the rehabilitation period.
In cases of cardiac insufficiency the drug increases myocardial contractility and tolerance to physical activity: reduces the frequency of angina attacks.
In acute and chronic ischemic disorders of cerebral circulation improves blood circulation in the focus of ischemia, redistributes blood in favor of the ischemic area.

Indications

For parabulbar administration: acute circulatory disorders in the retina, hemophthalmos and retinal hemorrhages of various etiologies, thrombosis of the central retinal vein and its branches, retinopathy of various etiologies (including diabetic and hypertensive) – only for parabulbar administration.

Pregnancy
During pregnancy and lactation (breastfeeding), the use of the drug is contraindicated.
Contraindications

Side Effects

Interaction

Enhances the effect of coronary dilatants, some antihypertensive drugs, cardiac glycosides.
When used together with nitroglycerin, nifedipine, alpha-blockers. other antihypertensive drugs and peripheral vasodilators should be used with caution due to the possible development of moderate tachycardia and arterial hypotension.
Overdose
Symptoms: decreased blood pressure, accompanied by headache, tachycardia, dizziness, and general weakness.
Treatment is symptomatic.
