Carboxym belongs to the group of pyridinium cholinesterase reactivators. It has a pronounced antidote effect in the treatment of lesions with organophosphorus compounds (OPs).
The timely use of carboxime significantly accelerates the recovery of cholinesterase activity inhibited by PHOS.
It also has a positive effect on some other manifestations of poisoning (reduces bronchial spasm, motor disorders, etc.).
Carboxym has low toxicity and practically no cholinolytic effect.
Indications
Carboxim is used against the background of antidotes (Atropine, Athene, P-10M) and other agents used in the treatment of poisoning with PHOS. After administration of the therapeutic antidote, carboxime is administered after 1-2 hours.

Composition
1 ml of solution for intramuscular injection contains:

auxiliary substances:
citric acid,
sodium hydroxide,
sodium chloride,
dinatrium edetate (trilon B),
water for injection.
How to take, the dosage
Intramuscularly 1 ml. The total dose of carboxime used in the care depends on the severity of the lesion.
In case of mild FOS lesions (restlessness, miosis, salivation, sweating) after administration of cholinolytic-type drugs, carboxym is used in a single therapeutic dose. In some cases, it is possible to reintroduce the drug in the same dose.
In moderate FOS lesions (agitation, myofibrillation, bronchospasm, cyanosis, tachycardia, rarely bradycardia), a double administration of carboxime in a single therapeutic dose with an interval of 2-3 hours will usually be required.
In severe FOS lesions (seizures, coma, respiratory and cardiovascular failure), the dose of carboxime may be increased to 3-4 ml. This dose is administered in 1 ml of the drug at intervals of 1-2 hours. Treatment of affected FOS with carboxime in all cases is carried out with systematic
monitoring of cholinesterase activity.
Contraindications
There are no contraindications to the use of Carboxym if there are symptoms of PHOS intoxication.
Side effects
In the recommended dose, Carboxym does not cause side effects. However, if high doses of Carboxym are used, there may be worsening of well-being, muscle weakness, dizziness, headache, nausea, and tachycardia.
Transient paresthesias may occur (mostly in the face).
The ECG may show phenomena of intracardiac conduction abnormalities and increased systolic rate. These phenomena soon go away and do not require additional treatment.
