Baclosan is an antispastic, myorelaxant.

Pharmacodynamics

By reducing the excitability of the end sections of the afferent sensory fibers and suppressing the intermediate neurons, inhibits mono- and polysynaptic transmission of nerve impulses; reduces the pretension of muscle spindles. It has no effect on neuromuscular transmission.

In neurological diseases accompanied with spasticity of skeletal muscles, weakens painful spasms and clonic convulsions; increases joint movement, facilitates passive and active kinesotherapy (physical exercise, massage, manual therapy).

Pharmacokinetics

After oral administration, Cmax in plasma is reached after 2-3 hours. Absorption is high. Protein binding is 30%. It penetrates through the placental barrier and is excreted with breast milk. It is metabolized in the liver. It is excreted by kidneys (mainly unchanged).

Indications

Increased muscle tone in multiple sclerosis;
diseases of the spinal cord of infectious, degenerative and traumatic origin (for example, syringomyelia tumors, motor neuron diseases, trauma);
stroke;
cerebral palsy;
meningitis;
traumatic brain injuries;
alcoholism (affective disorders).

For neurological diseases accompanied by spasticity of skeletal muscles, it weakens painful spasms and clonic convulsions; increases the range of motion in the joints, facilitates passive and active kinesitherapy (physical exercises, massage, manual therapy).

After oral administration, Cmax in blood plasma is reached within 2–3 hours. Absorption is high. Protein binding – 30%. Penetrates the placental barrier and is excreted in breast milk. Metabolized in the liver. Excreted by the kidneys (mainly unchanged).

Special instructions

In patients with liver disease and diabetes mellitus, it is necessary to periodically monitor the activity of liver transaminases, alkaline phosphatase, and blood glucose.

During treatment, you should refrain from potentially dangerous activities associated with the need for increased attention and rapid mental and motor reactions.

Side Effects

Nausea, vomiting, drowsiness, dizziness, increased fatigue, dry mouth, confusion, gait disturbance, urinary retention, constipation or diarrhea, decreased blood pressure, apathy; euphoria, depression, paresthesia, myalgia, muscle weakness, ataxia, tremor, nystagmus, accommodation paresis, hallucinations, dysuria, enuresis, convulsions, decreased seizure threshold.

With long-term use – impaired renal function.

Interaction

Enhances the effect of drugs that affect the central nervous system, antihypertensive drugs, ethanol.

Tricyclic antidepressants increase the decrease in muscle tone; Levodopa and cardipopa increase the risk of hallucinations, confusion, and agitation.

Patients should tell their doctor about all medications they are taking.

Overdose

Symptoms: muscle hypotension, depression of the respiratory center, confusion, coma (after the return of consciousness, muscle hypotension may persist for 72 hours).

Treatment: drinking plenty of fluids, taking diuretics, and in case of respiratory depression – mechanical ventilation. There is no specific antidote.
