Propionic acid derivative. It has analgesic, anti-inflammatory and antipyretic effects.
The mechanism of action is related to inhibition of COX activity – the main enzyme of arachidonic acid metabolism, which is the precursor of prostaglandins that play the main role in pathogenesis of inflammation, pain and fever.
The pronounced analgesic effect of ketoprofen is caused by two mechanisms: peripheral (indirectly, through inhibition of prostaglandin synthesis) and central (due to inhibition of prostaglandin synthesis in central and peripheral nervous system and also due to the effect on biological activity of other neurotropic substances which play key role in release of pain mediators in the spinal cord).
In addition, ketoprofen has anti-bradykinin activity, stabilizes lysosomal membranes, causes significant inhibition of neutrophil activity in patients with rheumatoid arthritis. Suppresses platelet aggregation.

Ketoprofen lysine (ketoprofen lysine salt) – 15 mg, which corresponds to the content of ketoprofen – 9.375 г.
Auxiliary substances:
Polysorbate 80 – 4 g,
propylene glycol – 4 g,
How to take, the dosage
It is set individually depending on the clinical situation.
It is used orally, intravenously (as an infusion), rectally, or externally in appropriate dosage forms.
Interaction
Concomitant use of ketoprofen with other NSAIDs increases the risk of gastrointestinal erosive ulcers and bleeding; with antihypertensive agents (including beta-adrenoblockers, ACE inhibitors, diuretics) – possible decrease of their action; with thrombolytics – increased risk of bleeding.
Concomitant use with acetylsalicylic acid may decrease the binding of ketoprofen to plasma proteins and increase its plasma clearance; with heparin, ticlopidine – increased risk of bleeding; with lithium preparations – possible increase of lithium concentration in blood plasma to toxic due to decrease of its renal excretion.
In concomitant use with diuretics there is increased risk of renal failure due to decreased renal blood flow caused by inhibition of prostaglandin synthesis and due to hypovolemia.
Concomitant use with probenecid may decrease clearance of ketoprofen and its binding to plasma proteins; with methotrexate – possible increase of side effects of methotrexate.
With concomitant use of warfarin severe, sometimes fatal bleeding is possible.
Special Instructions
Contraindications
For rectal use: proctitis and bleeding from the rectum in the history.
For external use: moist dermatoses, eczema, infected abrasions, wounds.
Side effects
Digestive system disorders: epigastric pain, nausea, vomiting, constipation or diarrhea, anorexia, gastralgia, liver function disorders; rarely – erosive-ulcerative gastrointestinal lesions, bleeding and gastrointestinal perforations.
CNS disorders: headache, dizziness, tinnitus, somnolence.
Urinary system disorders: renal dysfunction.
Allergic reactions: skin rash; rarely – bronchospasm.
Local reactions: when using in the form of suppositories irritation of the rectal mucosa and painful defecation are possible; when using in the form of gel – itching, skin rash at the application site.
Overdose
There are currently no reported cases of overdose with the drug Arthrosilen.
Pregnancy use
It should not be used in the third trimester of pregnancy.
There is no experience of using Arthrosilen during lactation.
The use in I and II trimesters is possible only after consultation with the doctor.
It is contraindicated for children under 6 years of age.
