A product that normalizes the energy metabolism of cells subjected to hypoxia or ischemia. It prevents reduction of intracellular ATP content, ensures normal functioning of membrane ion channels, transmembrane transport of potassium and sodium ions and preservation of cellular homeostasis.
Trimetazidine slows fatty acid oxidation through selective inhibition of long-chain 3-ketoacetyl-CoA thiolase, which leads to increased glucose oxidation and the restoration of conjugation between glycolysis and oxidative decarboxylation and has been shown to protect myocardium from ischemia. Switching fatty acid oxidation to glucose oxidation underlies the antianginal action of trimetazidine.
In experimental studies on animals it has been shown that trimetazidine maintains the energy metabolism of the heart and neurosensory organs during episodes of ischemia; it reduces the magnitude of intracellular acidosis and the degree of changes in the transmembrane ion flux that occurs during ischemia; Reduces the migration and infiltration of polynuclear neutrophils in ischemic and reperfused heart tissues, reduces the size of myocardial damage; while having no effect on hemodynamics.
