Analgesic-antipyretic. It is a derivative of pyrazolone. It has analgesic, antipyretic and anti-inflammatory effect, which mechanism is associated with inhibition of prostaglandin synthesis.
Pharmacokinetics
After oral administration sodium metamizole is rapidly hydrolyzed in the gastric juice to form the active metabolite 4-methyl-amino-antipyrine, which after absorption is metabolized to 4-formyl-amino-antipyrine and other metabolites.
After IV administration, sodium metamizole quickly becomes unavailable for determination.
The metabolites of sodium metamizole do not bind to plasma proteins. Most of the dose is excreted with the urine in the form of metabolites. Metabolites are excreted with breast milk.

Active ingredient: metamizole sodium – 0.5 g;
Excipients:
Potato starch, microcrystalline cellulose, colloidal silica (aerosil), hypromellose (hydroxypropyl methylcellulose), crosspovidone, magnesium stearate, talc; coating: Opadray II (polyvinyl alcohol, macrogol, talc, titanium dioxide, pigments (aluminum oxides).
How to take, the dosage
Orally or rectally, adults are prescribed 250-500 mg 2-3 times a day. Maximum single dose is 1 g, daily dose – 3 g.
The single dose for children aged 2-3 years is 50-100 mg; 4-5 years – 100-200 mg; 6-7 years – 200 mg; 8-14 years – 250-300 mg; number of times a day – 2-3 times.
I/m or intravenously slowly in adults – 250-500 mg 2-3 times per day. Maximum single dose is 1 g, daily dose – 2 g.
In children parenterally the dose is 50-100 mg per 10 kg of body weight.
Interaction
In concomitant use with analgesics-antipyretics, with NSAIDs mutual enhancement of toxic effects is possible.
Concomitant use with inducers of microsomal liver enzymes may reduce the effectiveness of sodium metamizole.
In concomitant use the activity of indirect anticoagulants, oral hypoglycemic drugs, GCS, indomethacin is increased due to their displacement from binding to blood proteins under the influence of sodium metamizole.
In concomitant use with phenothiazine derivatives pronounced hyperthermia is possible; with sedatives, anxiolytics – the analgesic effect of sodium metamizole is increased; with tricyclic antidepressants, oral contraceptives, allopurinol – metabolism of sodium metamizole is impaired and its toxicity increases; with caffeine – the effect of sodium metamizole is increased; with cyclosporine – concentration of cyclosporine in blood plasma decreases.
When applying sodium metamizole in combination with pitophenone hydrochloride (causes direct myotropic effect on smooth muscles of internal organs and causes its relaxation) and with phenpiperinia bromide (m-cholinoblocker) their pharmacological action is mutually reinforced, which is accompanied by pain relief, relaxation of smooth muscles and decrease of increased body temperature.
Special Instructions
In long-term use the peripheral blood picture should be monitored.
Sodium metamizole is used in combination with pitophenone and phenpiperinia bromide as an analgesic agent with antispasmodic effect.
Perhaps with caution in children in the first 3 months of life.
Contraindications
Side effects
Allergic reactions: skin rash, Quincke’s edema; rarely – anaphylactic shock.
Hematopoietic system: rare, with prolonged use – leukopenia, agranulocytosis.
Local reactions: in case of intravenous injection – infiltrates at the injection site.
