Amlodipine is a class II selective calcium channel blocker. Antihypertensive effect is due to direct relaxing effect on vascular smooth muscles.
It is assumed that antianginal action of amlodipine is related to its ability to dilate peripheral arterioles; it leads to decrease of ROP, reflex tachycardia does not occur.
The result is a decrease in myocardial oxygen demand and energy consumption by the heart muscle.
On the other hand, amlodipine appears to cause dilation of the large caliber coronary arteries and coronary arterioles of both intact and ischemic areas of myocardium. This ensures the flow of oxygen to the myocardium during coronary artery spasms.
Indications
Arterial hypertension (including as part of combination therapy), stable angina, unstable angina (including Prinzmetal’s angina)
Pharmacological effect

It is assumed that the antianginal effect of amlodipine is associated with its ability to dilate peripheral arterioles; this leads to a decrease in peripheral vascular resistance; reflex tachycardia does not occur.
As a result, there is a decrease in myocardial oxygen demand and energy consumption by the heart muscle.
On the other hand, amlodipine appears to cause dilation of large-caliber coronary arteries and coronary arterioles in both intact and ischemic areas of the myocardium. This ensures the supply of oxygen to the myocardium during spasms of the coronary arteries.
Special instructions
Amlodipine is used with caution in cases of liver dysfunction, aortic stenosis, and chronic heart failure.
Patients with impaired renal function, as well as elderly patients, do not require a dose reduction.
It is possible to use amlodipine for the treatment of patients with dilated (non-ischemic) cardiomyopathy, accompanied by a severe form of chronic heart failure.
There are no clinical data on the use of amlodipine in children.

Pregnancy
The safety of Amlodipine during pregnancy and lactation has not been established.
Use is possible only in cases where the expected benefit to the mother outweighs the potential risk to the fetus or child.
Contraindications
Severe arterial hypotension.
Hypersensitivity to dihydropyridine derivatives.
Side Effects
From the cardiovascular system: peripheral edema, palpitations, hyperemia of the skin are possible; when used in high doses – arterial hypotension, arrhythmias, shortness of breath.
From the digestive system: nausea, abdominal pain; rarely – gum hyperplasia.
From the side of the central nervous system: possible headache, fatigue, drowsiness, dizziness.
Allergic reactions: skin rash, itching.
Interaction
When Amlodipine is used simultaneously with other antihypertensive drugs, the hypotensive effect may be enhanced;
with drugs for inhalation anesthesia – the hypotensive effect may be enhanced;
with lithium preparations – manifestations of neurotoxicity are possible, incl. nausea, vomiting, diarrhea, ataxia, tremors and/or tinnitus;
with sympathomimetics – a decrease in the hypotensive effect is possible.
Indomethacin and other NSAIDs may reduce the hypotensive effect of amlodipine by inhibiting the synthesis of prostaglandins in the kidneys and/or causing fluid retention.
