Allopurinol

Composition

1 tablet contains allopurinol 300 mg

Auxiliary substances:

lactose monohydrate (milk sugar) – 49 mg,

microcrystalline cellulose – 20 mg,

sodium carboxymethyl starch (primogel) – 20 mg, <

gelatin – 5 mg,

magnesium stearate – 4 mg,

silica colloidal (aerosil) – 2 mg.

How to take, the dosage

Oral.
The drug should be taken once a day after meals with plenty of water. If the daily dose exceeds 300 mg or gastrointestinal intolerance symptoms are observed, the dose should be divided into several doses.

Allopurinol should be used in different dosages (100 mg) once a day for initial therapy. If this dose is not sufficient to properly reduce the uric acid concentration in the blood serum, the daily dose of the drug can be gradually increased until the desired effect is achieved. Special caution should be exercised in case of impaired renal function.

Interaction

Allopurinol increases the effect of coumarin anticoagulants, adenine arabinoside, and hypoglycemic drugs (especially with impaired renal function) when used concomitantly.

Special Instructions

Allopurinol should be used with caution in patients with hepatic and/or renal dysfunction (in both cases the dose should be reduced), hypothyroidism. During the initial period of allopurinol therapy, systematic evaluation of liver function parameters is necessary.

During allopurinol treatment, daily fluid intake should be at least 2 liters (under control of diuresis).

In the beginning of gout treatment, a flare-up of the disease may occur. For prevention it is possible to use NSAIDs or colchicine (0.5 mg 3). Note that with adequate therapy with allopurinol, large urate stones may dissolve in the renal pelvis and subsequently enter the ureter.

Asymptomatic hyperuricemia is not an indication for allopurinol use.

In children, allopurinol is used only in malignant neoplasms (especially leukemia) and in some enzyme disorders (Lesch-Nichen syndrome).

To correct hyperuricemia in patients with tumor diseases, allopurinol is recommended before starting treatment with cytostatics. In these cases, the minimum effective dose should be used. In addition, in order to reduce the risk of deposition of xanthines in the urinary tract, measures should be taken to maintain optimal diuresis and alkalinization of urine. If allopurinol and cytostatics are used concomitantly, more frequent monitoring of peripheral blood counts is necessary.

Allopurinol should not be taken while drinking alcohol.

Influence on driving and operating machinery: Use with caution in patients whose activities require high concentration and rapid psychomotor reactions.

Contraindications

Severe liver and/or renal dysfunction, pregnancy, lactation, hypersensitivity to allopurinol.

Side effects

Cardiovascular system: in single cases – arterial hypertension, bradycardia. Nausea, vomiting), diarrhea, transient increase of transaminase activity in serum; rarely – hepatitis; in single cases – stomatitis, liver dysfunction (transient increase of transaminase and alkaline phosphatase activity), steatorrhea.

CNS and peripheral nervous system: in single cases – weakness, fatigue, headache, dizziness, ataxia, somnolence, depression, coma, paresis, paresthesia, seizures, neuropathies, vision disorders, cataract, changes of optic papilla, disorders of sense of taste.

Blood system: in single cases – thrombocytopenia, agranulocytosis and aplastic anemia, leukopenia (most probable in patients with renal dysfunction).

Urinary system: rare – interstitial nephritis; in single cases – edema, uremia, hematuria.

Metabolism: in single cases – hyperlipidemia.

Allergic reactions: skin rash, hyperemia, itching, in some cases – angioimmune lymphadenopathy, arthralgia, fever, eosinophilia, fever, Stevens-Johnson syndrome, Lyell syndrome.

Dermatological reactions: in single cases – furunculosis, alopecia, hair discoloration.

Pregnancy use

It is contraindicated in pregnancy and during lactation (breast-feeding).
