Albumin is a preparation intended for parenteral nutrition, which is a partial substitute for blood plasma.
Indications
Burns, Bleeding Shock (traumatic, surgical, toxic); burns accompanied by dehydration and “clotting” of blood; acute blood loss; purulent-septic conditions; liver diseases (with impaired albumin-synthesizing function); kidney damage (nephritis, nephrotic syndrome).
Hypoproteinemia, hypoalbuminemia of various genesis, developing with alimentary dystrophy, plasma albumin content decreasing below 30 g/l, or colloid-oncotic pressure level decreasing below 15 mm Hg, or total protein decreasing below 50 g/l; GIT lesions with malabsorption or permeability disorders.
During surgeries using artificial circulation; therapeutic plasmapheresis; hemolytic disease in newborns during exchange blood transfusion.
During preoperative hemodilution and preparation of autologous blood components; in brain edema.
Active ingredient
Human albumin
Composition
- albumin derived from plasma for fractionation, 20%;
Auxiliary substances:
- sodium caprylic acid (5.7-6.4) g/l;
- sodium chloride (90-160) mmol/l; – water for injection up to 1 liter.
.
How to take, the dosage
Albumin solution 20% – intravenously by drip at a rate of not more than 40-50 drops per minute, single dose is determined individually, taking into account the age and severity of the patient.
The elderly should avoid using concentrated albumin solutions of 20% because it can lead to an overload of the cardiovascular system.
Jet injection of albumin solution is acceptable in shocks of various genesis for rapid increase in blood pressure.
Interaction
A separate system is used for infusion of albumin solution. In cases of severe blood loss, the use of albumin solution can be combined with blood transfusion.
Special Instructions
Before use, you must make a visual inspection of the drug and packaging: the solution should be transparent, without any inclusions, the glass packaging should be hermetically sealed, without cracks.
The label data (drug name, manufacturing company, series number) should be recorded in the case history.
Administration of the drug in case of dehydration is possible only after prior provision of sufficient fluid intake (internally and parenterally). There is no data concerning the use of albumin during lactation.
When using drugs prepared from human plasma the risk of transmission of known and as yet unknown viral infections cannot be completely excluded.
Caution is exercised when cardiac function is inhibited (due to the possible occurrence of acute heart failure).
Contraindications
Hypersensitivity, hypervolemia, IIB-III degree heart failure, pulmonary edema, severe anemia.
With caution: renal failure, bleeding, thrombosis, arterial hypertension.
Side effects
Introduction of albumin solution, as a rule, is not accompanied by adverse reactions and complications. Some previously sensitized patients may have allergic reactions of varying severity. Possible (extremely rare) adverse reactions and complications are not excluded in patients belonging to risk group, i.e. those with history of intravenous intolerance to protein preparations and other plasma substitutes, drugs, serums and vaccines.
In case of reactions or complications, immediately stop transfusion of albumin solution and, without removing the needle from the vein, administer antihistamines, cardiotonic drugs, glucocorticosteroids, vasopressor drugs (if indicated).
