It has anti-inflammatory, analgesic and antipyretic effects associated with the inhibition of cyclooxygenase 1 and 2, which regulate the synthesis of prostaglandins.

How to take, the dosage
Interaction
In concomitant use, antacids containing magnesium and/or aluminum hydroxide slow down and reduce absorption of acetylsalicylic acid.
In concomitant use of calcium channel blockers, means limiting the intake of calcium or increasing the excretion of calcium from the body, the risk of bleeding increases.
Concomitant use with acetylsalicylic acid increases the effect of heparin and indirect anticoagulants, hypoglycemic agents sulfonylurea derivatives, insulin, methotrexate, phenytoin, valproic acid.
Concomitant use with GCS may increase the risk of ulcerogenic effects and gastrointestinal bleeding.
The effectiveness of diuretics (spironolactone, furosemide) decreases with concomitant use.
Concomitant use of other NSAIDs increases the risk of side effects. Acetylsalicylic acid may decrease the plasma concentrations of indomethacin, piroxicam.
Accetylsalicylic acid may induce liver damage when used concomitantly with gold preparations.
The effectiveness of uricosuric agents (including probenecid, sulfinpyrazone, benzbromaron) decreases with concomitant use.
Concomitant use of acetylsalicylic acid and sodium alendronate may cause severe esophagitis.
Concomitant use of griseofulvin may impair absorption of acetylsalicylic acid.
A case of spontaneous iris hemorrhage has been described when taking ginkgo biloba extract with long-term use of acetylsalicylic acid at a dose of 325 mg/day. It is believed that this may be due to the additive inhibitory effect on platelet aggregation.
Concomitant use of dipyridamole may increase plasma Cmax and AUC of salicylate.
Concomitant use with acetylsalicylic acid increases plasma concentrations of digoxin, barbiturates and lithium salts.
In concomitant use of salicylates in high doses with carboangidase inhibitors, intoxication with salicylates is possible.
Acetylsalicylic acid in doses less than 300 mg/day has little effect on the effectiveness of captopril and enalapril. When acetylsalicylic acid is used in high doses, the efficacy of captopril and enalapril may be reduced.
Caffeine increases the rate of absorption, plasma concentrations and bioavailability of acetylsalicylic acid when used concomitantly.
Concomitant use of metoprolol may increase the Cmax of salicylate in plasma.
When pentazocine is used with prolonged use of acetylsalicylic acid at high doses, there is a risk of severe adverse reactions from the kidneys.
Phenylbutazone reduces uricosuria caused by acetylsalicylic acid when used concomitantly.
When used concomitantly, ethanol may increase the effect of acetylsalicylic acid on the gastrointestinal tract.
Special Instructions
With caution, use in patients with liver and kidney disease, with bronchial asthma, erosive-ulcerative lesions and bleeding from the gastrointestinal tract in the history, with increased bleeding or with concomitant anticoagulation therapy, decompensated chronic heart failure.
Acetylsalicylic acid, even in small doses, reduces the excretion of uric acid from the body, which may cause an acute attack of gout in predisposed patients. During long-term therapy and/or when using acetylsalicylic acid in high doses, medical supervision and regular monitoring of hemoglobin levels are required.
The use of acetylsalicylic acid as an anti-inflammatory agent in a daily dose of 5-8 g is limited due to the high possibility of gastrointestinal side effects.
Before surgery, to reduce bleeding during surgery and in the postoperative period, salicylates should be discontinued 5-7 days in advance.
During prolonged therapy, a total blood count and fecal occult blood test should be performed.
The use of acetylsalicylic acid in pediatrics is contraindicated because in case of viral infection in children under the influence of acetylsalicylic acid the risk of Reye’s syndrome increases. Symptoms of Reye syndrome are prolonged vomiting, acute encephalopathy, and enlarged liver.
The duration of treatment (without consulting a doctor) should not exceed 7 days when prescribed as an analgesic and more than 3 days as an antipyretic.
The patient should refrain from drinking alcohol during treatment.
Contraindications
With caution – hyperuricemia, urate nephrolithiasis, gout, gastric and/or duodenal ulcer (in anamnesis), decompensated heart failure.
Side effects
Gastrointestinal tract: abdominal pain, heartburn, nausea, vomiting, overt (vomiting with blood, tarry stools) or latent signs of gastrointestinal bleeding, which may lead to iron deficiency anemia, erosive and ulcerative lesions (including with perforation) of the gastrointestinal tract, single cases – liver function disorders (increased liver transaminases).
Central nervous system: dizziness, tinnitus (usually a sign of overdose).
Blood system disorders: increased risk of bleeding due to the effect of acetylsalicylic acid on platelet aggregation.
Allergic reactions: skin rash, anaphylactic reactions, bronchospasm, Quincke’s edema.
If these symptoms occur, it is recommended that you stop taking the drug and see your doctor immediately.
Overdose
Symptoms: Moderate overdose: nausea, vomiting, tinnitus, hearing loss, headache, dizziness, and confusion. These symptoms disappear with reduction of the drug dose.
Severe overdose: fever, hyperventilation, ketoacidosis, respiratory alkalosis, metabolic acidosis, coma, cardiogenic shock, respiratory failure, severe hypoglycemia.
Treatment:Hospitalization, gastric lavage, administration of activated charcoal, monitoring of acid-base balance, alkaline diuresis to obtain urine pH between 7.5-8 (forced alkaline diuresis is considered achieved if plasma salicylate concentration is more than 500 mg/L (3.6 mmol/L) in adults or 300 mg/L (2.2 mmol/L) in children), hemodialysis, fluid loss compensation, symptomatic therapy.
