Theotard is theophylline (a xanthine derivative) in pellet form. Theophylline belongs to phosphodiesterase inhibitors, increases accumulation of c-AMP in tissues, blocks adenosine (purine) receptors; reduces calcium ions entry through cell membrane channels, reduces smooth muscle contractile activity. It relaxes the muscles of the bronchi and blood vessels (mainly vessels of the brain, skin and kidneys); it has a peripheral vasodilator effect, increases renal blood flow, has a moderately pronounced diuretic effect.

Stabilizes the membrane of mast cells, inhibits the release of mediators of allergic reactions. Increases mucociliary clearance, stimulates contraction of the diaphragm, improves the function of the respiratory and intercostal muscles, and stimulates the respiratory center. Normalizes respiratory function, promotes blood oxygenation and reduces carbon dioxide concentration; stimulates respiratory centers. Increases lung ventilation in conditions of hypokalemia.

It has a stimulating effect on heart activity, has positive inotropic and chronotropic effect on the heart, increases coronary blood flow and myocardial oxygen demand. It reduces the tone of blood vessels (mainly vessels of the brain, skin and kidneys).

Decreases pulmonary vascular resistance, decreases pressure in the small circle of the circulation. Increases renal blood flow, has a moderate diuretic effect. Dilates extrahepatic biliary tracts. Inhibits platelet aggregation (inhibits platelet activation factor and PgE2 alpha), increases resistance of erythrocytes to deformation (improves rheological properties of blood), reduces thrombosis and normalizes microcirculation.

The substituted release of active substance from sustained release capsules ensures reaching therapeutic level of theophylline in blood 3-5 hours after intake and its maintenance for 12 hours, so that effective theophylline concentrations in blood during 24 hours are maintained when taking the drug 2 times a day.

The bronchodilator effect of Theotard develops gradually, so the drug is not prescribed to relieve emergency conditions.

How to take, the dosage

Interaction

Theophylline is not used together with other xanthine derivatives. In combination with phenobarbital, diphenin, rifampicin, isoniazid, carbamazepine, sulfinpyrazone, phenytoin the effectiveness of theophylline is reduced, which may require increasing the used doses of the drug.

In concomitant use with antibiotics of the macrolide group, lincomycin, allopurinol, cimetidine, isoprenaline, oral contraceptives, enoxacin and during vaccination against influenza the intensity of action may increase, which may require reducing the dose of theophylline.

The drug suppresses the therapeutic effects of lithium carbonate and beta-adrenoblockers. Theophylline potentiates the effects of diuretics by increasing glomerular filtration and decreasing tubular reabsorption. With caution, theophylline is prescribed simultaneously with anticoagulants.

Special Instructions

Theotard is not recommended for use in arterial hypertension, widespread vascular atherosclerosis, marked liver and kidney function disorders.

Overdose

Symptoms: overdose of the drug may cause stomach pain, nausea, vomiting, diarrhea, headache, abnormal heart rhythm, tachycardia, decreased blood pressure, increased angina pectoris, insomnia, tremor, irritability, increased diuresis, hyperglycemia, hypokalemia in some patients. Skin rash and itching are possible.

Treatment: In these cases the dose should be reduced.
