SUPRAX is a hemisynthetic broad-spectrum cephalosporin antibiotic of III generation for oral administration. It acts bactericidally. Mechanism of action is caused by inhibition of pathogen cell membrane synthesis. Cefixime is resistant to β-lactamases produced by majority of Gram-positive and Gram-negative bacteria.
In vitro and in clinical practice cefixime is active against Gram-positive bacteria: Streptococcus pneumoniae, Streptococcus pyogenes; against Gram-negative bacteria: Haemophilus influenzae, Moraxella (Branhamella) catarrhalis, Escherichia coli, Proteus mirabilis, Neisseria gonorrhoeae.
In vitro cefixime is active against Gram-positive bacteria: Streptococcus agalactiae; Gram-negative bacteria: Haemophilus parainfluenzae, Proteus vulgaris, Klebsiella pneumoniae, Klebsiella oxytoca, Pasteurella multocida, Providencia spp, Salmonella spp., Shigella spp., Citrobacter amalonaticus, Citrobacter diversus, Serratia marcescens.
Pseudomonas spp, Enterococcus (Streptococcus) serogroup D, Listeria monocytogenes, Staphylococcus spp. (including methicillin-resistant strains), Enterobacter spp., Bacteroides fragilis, Clostridium spp.
Indications
Infectious and inflammatory diseases caused by microorganisms sensitive to the drug:
– pharyngitis;
– tonsillitis;
– sinusitis;
– acute and chronic bronchitis;
Active ingredient
Composition
Cefixime (in the form of trihydrate)
Excipients:
colloidal silicon dioxide,
magnesium stearate,
calcium carmellose.
How to take, the dosage
For adults and children over 12 years of age with body weight over 50 kg the daily dose is 400 mg once a day. The course of treatment is 7-10 days.
In uncomplicated gonorrhea 400 mg is prescribed once.
Children under 12 years of age are prescribed in the form of suspension in a dose of 8 mg/kg of body weight once/day or 4 mg/kg every 12 hours.
In case of infections caused by Streptococcus pyogenes the daily dose is 6-10 ml of suspension, in 2-4 year-old children – 5 ml, in 6-month-old children – 2.5-4 ml.
In renal dysfunction (at IK from 21 to 60 ml/min) or in patients on hemodialysis the daily dose should be reduced by 25 %.
In IK â¤20 ml/min or in patients on peritoneal dialysis the daily dose should be reduced twice.
In patients with impaired renal function with IQ less than 60 ml/min or in patients on hemodialysis or peritoneal dialysis the drug should be administered in form of suspension.
Rules for preparation of suspension
Turn the bottle upside down and shake the pellets. Add 40 ml of cooled to room temperature boiled water in 2 steps and shake after each addition until a homogeneous suspension is formed. After that, let the suspension stand for 5 min to ensure complete disintegration of granules. The ready to use suspension should be shaken before use.
Interaction
Blockers of tubular secretion (allopurinol, diuretics) delay excretion of cefixime by the kidneys, which may lead to increased concentrations of the drug in plasma.
With the simultaneous use of cefixime and carbamazepine the concentration of carbamazepine in blood plasma increases.
Decreases prothrombin index, increases the effect of indirect anticoagulants.
Antacids containing magnesium or aluminum hydroxide slow down the absorption of the drug, so the drug should be used 1-2 hours before or 4 hours after taking these medicines.
Contraindications
– renal impairment with CKR less than 60 ml/min (for capsules);
– childhood under 12 years (for capsules);
– hypersensitivity to cephalosporins and penicillins.
Suprax shall be administered with caution in elderly patients with chronic renal insufficiency or pseudomembranous colitis (in anamnesis), in pellet form for suspension preparation – in children under 6 months of age.
Side effects
Digestive system: dry mouth, anorexia, nausea, vomiting, diarrhea, constipation, abdominal pain, flatulence, transient increase in liver transaminases and alkaline phosphatase activity, hyperbilirubinemia, jaundice, gastrointestinal candidiasis, dysbacteriosis; rarely – stomatitis, glossitis, pseudomembranous enterocolitis.
Hematopoietic system: leukopenia, thrombocytopenia, neutropenia, hemolytic anemia, pancytopenia, aplastic anemia, bleeding.
CNS disorders: dizziness, headache.
Urinary system disorders: interstitial nephritis, renal dysfunction, acute renal failure.
Allergic reactions: skin itching, urticaria, skin hyperemia, eosinophilia, fever, erythema multiforme, toxic epidermal necrolysis, anaphylactic shock.
Laboratory findings: increased urea nitrogen, hypercreatininemia, increased prothrombin time.
Other: candidiasis, dyspnea, development of hypovitaminosis B.
Overdose
Treatment: gastric lavage; symptomatic and supportive therapy, which if necessary includes the use of antihistamines, GCS, epinephrine, norepinephrine, dopamine, oxygen therapy, transfusion of infusion solutions, EVI. Cefixime is not excreted in large quantities from the circulating blood by hemo- or peritoneal dialysis.
Pregnancy use
The use of Suprax in pregnancy is possible only if the estimated benefit to the mother exceeds the potential risk to the fetus.
If Suprax has to be used during lactation, breastfeeding should be stopped.
