Pharmacodynamics

Antimicrobial bacteriostatic agent. Mechanism of action is due to antagonism with paraaminobenzoic acid (PABA) and competitive inhibition of dihydropteroatsinthetase, which leads to disruption of tetrahydrofolic acid synthesis, necessary for purine and pyrimidine synthesis.

It inhibits the growth of E. coli and reduces the synthesis of thiamine, riboflavin, nicotinic acid in the intestine. SULGIN has bacteriostatic effect on Streptococcus spp., Staphylococcus spp., Escherichia coli, Shigella dysenteriae, Proteus vulgaris.

Pharmacokinetics

After oral administration, the drug is slightly absorbed from the digestive tract, resulting in high bacteriostatic concentration in the intestine.

How to take, the dosage

In acute dysentery there are 2 courses of treatment.

The first course: on days 1 and 2 – 1 g every 4 hours (6 g/day total), days 3 and 4 – 1 g every 6 hours (4 g/day), days 5 and 6 – 1 g every 8 hours (3 g/day).

If necessary, 5-6 days later, the second course is carried out: 1 and 2 days – 1 g every 4 hours, at night – 8 hours (5 g/day total), 3 and 4 days – 1 g every 4 hours (4 g/day), at night do not give, 5 day – 3 g/day. Children over 3 years of age – 0.5 g 4 times a day for 7 days.

In the treatment of other infections: adults – orally, 1-2 g every 4-6 hours; on the next 2-3 days – half doses. For children over 3 years old – on the first day 0.1 g/kg/day in equal doses every 4 hours, on the following days – 0.25-0.5 g every 6-8 hours.

For prevention of postoperative intestinal complications – 0.05 g/kg every 8 hours, for 5 days before and 7 days after surgery.

The highest doses for adults: single dose 2 g, daily – 7 g.

Interaction

Decreases the reliability of oral contraception.

Benzocaine, procaine reduce antimicrobial activity (during hydrolysis they release PABA).

Non-steroidal anti-inflammatory drugs, hypoglycemic drugs (sulfonylurea derivatives), phenytoin and indirect anticoagulants increase the severity of the side effects of the drug.

Ascorbic acid, hexamethylantetramine increase the risk of crystalluria.

Myelotoxic drugs increase the manifestation of hematotoxicity of the drug.
