Pyroxicam is an analgesic, antipyretic, anti-inflammatory.
Selectively inhibits cyclooxygenase, disrupts the metabolism of arachidonic acid, reduces the formation of GH.
Allows or eliminates moderate pain, reduces the severity of inflammation, in joint syndrome reduces morning joint stiffness, helps to increase range of motion.
The full therapeutic effect is seen after 10-14 days of use.
Indications
Rheumatoid arthritis, osteoarthritis, spondylitis, acute gout attack, inflammatory diseases of the musculoskeletal system.

How to take, the dosage
Overly, 20 to 40 mg daily in 1 to 2 doses.
Interaction
Enhances the effect of oral anticoagulants, increases the concentration of lithium salts in the blood.
Alcohol increases the likelihood of side effects.
Blood levels are reduced by salicylic acid derivatives.
Special Instructions
Pyroxicam should be prescribed by physicians experienced in the diagnosis and treatment of inflammatory and degenerative joint diseases.
Concomitant use with misoprostol, proton pump inhibitors, H2-histamine receptor blockers reduces gastrointestinal toxicity.
NSAIDs, including piroxicam, can cause severe gastrointestinal side effects, including bleeding, ulceration and perforation of the stomach, small and large intestine (including fatal). These phenomena can occur at any time of treatment and are not even accompanied by threatening symptoms. The risk of these phenomena is particularly high for piroxicam; it is increased in the elderly.
The use of piroxicam significantly increases the risk of severe skin reactions (exfoliative dermatitis, Stevens-Johnson syndrome, toxic epidermal necrolysis) especially in the early stages of treatment. If the first signs of rash, mucosal ulceration or other signs of hypersensitivity occur, pyroxicam should be immediately discontinued.
Piroxicam, as well as other NSAIDs, increases the risk of thrombosis. NSAIDs increase the risk of thrombotic complications (including myocardial infarction and stroke), patients with risk factors are prescribed these drugs with caution.
Pyroxicam, as well as other NSAIDs, may contribute to the occurrence of thrombotic complications. In women the effect of piroxicam on myometrial tone may decrease the contraceptive effect of IUDs.
During treatment, ethanol should be avoided.
During treatment, caution should be exercised while driving vehicles and engaging in other potentially dangerous activities that require increased concentration and quick psychomotor reactions.
Contraindications
Hypersensitivity, including to NSAIDs, peptic ulcer disease, bleeding disorders, liver and kidney disease, cardiovascular failure, bronchial asthma, pregnancy, breast-feeding (breast-feeding should be stopped).
Side effects
Erotic ulcerative lesions of the gastrointestinal tract, edema, headache, dizziness, drowsiness, allergic skin reactions.
Overdose
Symptoms: nausea, vomiting, abdominal pain, somnolence, lethargy; GI bleeding, increased BP, acute renal failure, respiratory depression, coma.
Treatment: symptomatic. There is no specific antidote. In case of recent overdose it is necessary to induce vomiting, take osmotic laxatives, activated charcoal (60-100 g in adults, 1-2 g/kg in children). Multiple use of activated charcoal (6 hours after the intake) reduces T1/2 of piroxicam by 50% and its bioavailability by 37%.
Forced diuresis, urine alkalinization, hemodialysis and hemoperfusion are ineffective (high plasma protein binding).
