Antithyroid. It blocks peroxidase, disrupts iodination and formation of di-, tri- and tetraiodothyronine.
Pharmokinetics
It is well absorbed from the gastrointestinal tract and quickly metabolized.
About 35% is excreted in the urine unchanged and as conjugates within 24 hours.
Extracted into breast milk (concentration in breast milk is 10 times lower than in mother’s blood), it may cause hypothyroidism in infants whose mothers received propylthiouracil in therapeutic doses while breastfeeding.

How to take, the dosage
It is established individually depending on the severity of hyperthyroidism.
The average single dose is 100-300 mg; the frequency of administration is 3-6 times a day, after meals.
The average daily dose for children from 6 to 10 years is 50-150 mg/day, over 10 years – 150-300 mg/day.
The duration of therapy is 1-1.5 years.
Interaction
If Propicil® is treated with a “block and replace” regimen in combination with thyroxine, the dose of Propicil® must be increased.
The thyrostatic effect of Propicil® is reduced with concomitant or prior use of iodine-containing drugs or radiopaque agents.
Because of the ability of propylthiouracil to affect the effect of free active fractions of propranololol and coumarin derivatives, additional dose adjustments of these drugs are necessary.
Special Instructions
Treatment is carried out under control of the level of thyroid hormones, morphological blood picture, the level of activity of liver transaminases, bilirubin concentration, alkaline phosphate.

Side effects
Rarely – agranulocytosis, accompanied by septic complication of erythropoiesis disorders, hemolysis.
Very rarely – thrombocytopenia vasculitis, lupus-like syndrome, nodular periarteritis.
Skin rashes, urticaria.
Very rare – hypersensitivity reactions.
Dizziness , neuromuscular disorders, disorders of taste sensitivity and smell.
Nausea, vomiting, stomach pain.
very rarely – liver damage: hepatitis-like reactions with hepatocytic necrosis, transient cholestasis.
Thyroid enlargement, goiter formation in the newborn, arthralgia without signs of joint inflammation, interstitial pneumonia, peripheral edema, alopecia, positive Coombs reaction.
very rarely – drug-induced fever, lymphadenopathy.
Overdose
No cases of acute intoxication have been observed. Chronic overdose leads to goiter and hypothyroidism with symptoms depending on the severity of hypothyroidism due to the specific action of the antithyroid agent.
Treatment: there are no special measures/interventions. Gastric lavage and endoscopic removal of pill residues are not effective due to rapid absorption of the active ingredient.
In case of development of goiter and hypothyroidism accompanied by symptoms correlating with the severity of hypothyroidism and caused by chronic overdose of the drug, treatment with Propicil® is stopped and spontaneous recovery of thyroid function is expected. Thyroxine replacement therapy is carried out if justified by the severity of hypothyroidism.
Pregnancy use
It is contraindicated in pregnancy, during lactation.
Paediatric use
Paediatric use is possible in children over 6 years of age according to the dosing regimen.
