Pharmacological properties
Pharmacodynamics
Benzalconium chloride is both a spermicide and an antiseptic. The spermicidal effect is due to the ability of the active substance to destroy the membranes of sperm cells (first flagella, then heads), which leads to the inability to fertilize the egg with damaged sperm.
The effect develops immediately after the introduction of the cream in the vagina.
The use of Farmatex significantly reduces the risk of pregnancy, but does not eliminate it completely.
Pharmatex has no effect on normal vaginal microflora (including Döderlein bacillus).
Pharmatex has no effect on the menstrual cycle, libido or fertility.
In vitro benzalkonium chloride is active against Neisseria gonorrhoeae, Chlamydia spp., Trichomonas vaginalis, Herpes simplex type 2, Staphylococcus aureus. It has no effect on Mycoplasma spp. and weak effect on Gardnerella vaginalis, Candida albicans, Haemophilus ducreyi and Treponema pallidum.
Pharmacokinetics
The absorption of benzalkonium chloride by the vaginal mucosa is very low. The drug is removed from the mucosal surface by simple washing with water and normal physiological secretions.
Indications
Local contraception for women of reproductive age, and especially:
– if there are contraindications to the use of hormonal contraceptives or intrauterine contraception;
– in the postpartum period, during breastfeeding; after pregnancy termination;
– in premenopause; with irregular sexual intercourse;
– As additional local contraception when using a vaginal diaphragm, cervical cap, or other intrauterine contraceptives, especially with long-term use of medications such as NSAIDs (non-steroidal anti-inflammatory drugs).
As an additional method of contraception if you miss or are late in taking oral hormonal contraceptives. In this case, it is recommended to combine both methods of contraception for the remaining days of the menstrual cycle.

Composition
Composition per 100 g of cream
The active ingredient:
Benzalkonium chloride 50% aqueous solution – 2.4 g,
which corresponds to benzalkonium chloride – 1.2 g
Excipients:
citric acid – 0.002 g
dodecahydrate disodium hydrophosphate – to pH 3.5-5.5
macrogol and ethylene glycol
Palmitostearate (Tefoz® 63) – 24.0 g
Lavender oil – 0.05 g
Purified water – up to 100 g.
How to take, the dosage
For vaginal use.
Push the dispenser onto the tube instead of the cap. Gently squeeze out the contents of the tube until the pipette is full (to the stop of the piston) so that no air bubbles form. Remove the dispenser from the tube. Close the tube with the cap.
Lying on your back, apply the cream deep into the vagina using the dispenser, slowly pressing the plunger. The action time is 10 hours. Always make sure to add a new dose of the cream before each sexual intercourse.
One dose (5 g of cream) is for one sexual intercourse.
Interaction
Any product used intravaginally may reduce the spermicidal effect of the drug. The active ingredient, benzalkonium chloride, is destroyed by soap, so it is prohibited to use soap for genital toiletting, even in minimal quantities.
Special Instructions
Pharmatex is an effective contraceptive when used correctly. However, the spermicidal topical contraceptive method is always less effective than hormonal contraception, intrauterine devices, diaphragms, cervical caps or condoms.
The use of soap to wash the genitals is prohibited
2 hours before and 2 hours after intercourse, because soap destroys the active ingredient of Farmatex.
Toilet the external genitalia is possible only with plain water for both partners. Vaginal irrigation (douches) can be used not earlier than 2 hours after sexual intercourse.
With Farmatex injected into the vagina, you should not take a bath or swim in the sea, pools or bodies of water to avoid reducing its contraceptive effect.
If treatment with vaginal products is necessary, Pharmatex should be temporarily discontinued until the end of treatment.
This method of contraception does not protect against sexually transmitted infections or those caused by the human immunodeficiency virus. The only barrier contraceptive method you can use against these diseases is the condom.
Influence on driving ability
There are no data on adverse effects of the drug on the ability to drive vehicles and other mechanisms.

Contraindications
Side effects
Allergic reactions, contact dermatitis, itching and burning in the vagina and/or the partner’s penis, painful urination.
If these occur, discontinue use of Farmatex.
If any of the side effects listed in the instructions worsen, or if you notice any other side effects not listed in the instructions, tell your doctor.
Overdose
There have been no recorded cases of overdose.
Pregnancy use
Pregnancy
The drug is a contraceptive and is not used during pregnancy.
Inadvertent use of this spermicide in early pregnancy has not shown any adverse effects on the fetus.
Breastfeeding
Infusion into breast milk is possible in very small amounts without any known adverse effects. The drug can be used during breastfeeding.
