Pharmgroup:
an alpha-adrenomimetic.
Pharmic action:
Noradrenaline Agetan is an αl- and α2-adrenergic receptor agonist.
It weakly excites β1- and has almost no effect on β2-adrenoreceptors.
It has a powerful vasoconstrictor effect and leads to increased BP.
Indications
Acute hypotension accompanying cardiovascular collapse and shock (to restore and maintain blood pressure – intravenous perfusion).

Norepinephrine Agetan is an agonist of αl- and α2-adrenergic receptors.
It weakly excites β1-adrenergic receptors and has virtually no effect on β2-adrenergic receptors.
It has a powerful vasoconstrictor effect, leading to an increase in blood pressure.

Composition
Concentrate Norepinephrine Agetan for the preparation of solution for intravenous administration
1 ml:
– norepinephrine tartrate monohydrate (in terms of norepinephrine tartrate) 2 mg/ml
excipients:
sodium chloride;
sodium hydroxide or hydrochloric acid (pH = 3.0–4.5);
water for injections.
Pregnancy
Pregnancy is not a limitation for the use of Noradrenaline Agetan.
Breastfeeding should be stopped during treatment (there is no experience of using the drug in lactating women).
Contraindications
severe hypersensitivity to the drug;
arterial hypotension caused by hypovolemia. An exception in these cases is the need to administer the drug to maintain blood flow in the coronary arteries of the heart and arteries of the brain until the end of therapy aimed at restoring bcc;
thrombosis of mesenteric and peripheral vessels due to the risk of worsening ischemia and increasing the infarction area;
carrying out fluorotane and cyclopropane general anesthesia (due to the risk of developing ventricular tachycardia and fibrillation);
severe hypoxia and hypercapnia.
With caution:
severe left ventricular failure, acute heart failure, recent myocardial infarction;
simultaneous use of MAO inhibitors of the imipramine and triptyline series (due to the risk of developing a pronounced and prolonged increase in blood pressure);
development of symptoms of cardiac arrhythmia (in this case, a reduction in the dose of the drug is necessary). In case of polymorphic extrasystoles or the development of volleys of extrasystoles, the dose of the drug should be significantly reduced or treatment should be discontinued.
Side Effects
When administering Norepinephrine Agetan, the following symptoms may occur:
ischemic disorders up to necrosis, caused by vasoconstriction and tissue hypoxia, as well as due to extravasation;
arrhythmia;
reflex bradycardia;
anxiety;
difficulty breathing;
tremor;
transient headache;
retrosternal or pharyngeal pain;
autonomic reactions – pallor, increased sweating, vomiting, tachycardia. In particularly sensitive patients, an increase in blood pressure may be accompanied by headache, photophobia, stabbing chest pain, pallor of the skin, increased sweating and vomiting.
With long-term use, dose adjustment is necessary to avoid recurrent hypotension when the drug is discontinued.
Interaction
The main undesirable effects when combined with certain groups of drugs are pharmacodynamic.
When combined with inhalational anesthetics, serious ventricular arrhythmias (increased excitability of the heart) can occur.
When combined with serotonergic and adrenergic antidepressants, as well as imipramine and amitriptyline derivatives, paroxysmal hypertension may develop with a risk of cardiac arrhythmia due to the adrenergic blocking effect on the sympathetic nervous system.
The combination of Noradrenaline Agetan with selective and non-selective MAO inhibitors, linezolid is contraindicated, because in this case, a synergistic increase in the pressor effect of norepinephrine occurs, which requires careful medical monitoring.
Overdose
Symptoms: spasm of skin vessels, collapse, anuria, and a marked increase in blood pressure may be observed.
Treatment: if overdose occurs, the dose of the drug should be reduced.
