The active ingredient of Norcalut, norethisterone, is basically a norsteroid of the gestagenic type. Compared to other known gestagens, although it is not one of the most active gestagens, its properties are unusually favorable for therapy.

In the so-called “delay of menses” test, which is used to determine the effectiveness of gestagens, the drug shows properties of weak gestagens. The compound was found to be a medium-intensity progestive steroid in the Hammerstein test, as well as in the Clauberg test, which measures the dose required to transform the endometrium.

In contrast to many gestagenic compounds, norethisterone also has a pronounced anti-estrogenic character, and unlike other norethisterone derivatives, only a small portion of the compound is converted to estrogen in the body.

At the same time, Paulsen observed gynecomastia in men and breast enlargement in women under the influence of norethisterone. In addition to the main gestagenic action, a weak androgenic property of the compound has also been determined. According to clinical-pharmacological studies, norethisterone blocks the production of follicle stimulating hormone (FSH) and corpus luteum hormone (LH) and reduces steroid synthesis by the corpus luteum.

Norethisterone, which has a mainly gestagenic property, showing significant anti-estrogenic and minimal estrogenic and androgenic effects, can be very widely used in obstetrics and gynecology.

Indications

Premenstrual syndrome, mastodynia, cycle disorders accompanied by a shortening of the secretion period, dysfunctional uterine bleeding, cystic glandular endometrial hyperplasia, endometriosis, adenomyosis, cessation and prevention of lactation, delay of menstruation, menopause, in case of individual need, oral contraception.

Side Effects

Rarely encountered and passing in subsequent cycles: nausea, gastrointestinal complaints, extraordinary bleeding, sometimes swelling, allergic skin reaction, paresthesia, weight change, fatigue.

Interaction

Drugs that activate microsomal liver enzymes (barbiturates, etc.) accelerate metabolism and weaken the effect;

on the contrary, inhibitors (cimetidine, etc.) enhance it.
