Pharmacological properties
Pancreatin is a powder from the pancreas of pigs.
Pancreatic enzymes (lipase, amylase, protease) have proteolytic, amylolytic and lipolytic effects and contribute to the breakdown of proteins, fats, carbohydrates, improve the functional state of the gastrointestinal tract (GIT), thus normalizing the digestive processes. It is assumed to inhibit active trypsin by feedback type of stimulated pancreatic secretion in the upper part of the small intestine. Analgesic effect of pancreatin preparations described in some cases is attributed to this effect. Pancreatic enzymes are not absorbed in the gastrointestinal tract and are excreted through the intestine, most of them being denatured by digestive juices.

- Pancreatic enzymatic (exocrine) function disorders in diseases of the gastrointestinal tract with digestive disorders.

Pancreatin* – 116.66 – 134.61 mg with minimal activity:
- lipase – 3500 units Ph.Eur.
- amylase 4200 units Ph.Eur.
- protease 250 units Ph.Eur.
Auxiliary substances:Microcrystalline cellulose (type 12), sodium carboxy-methyl starch (type A), colloidal anhydrous silica, magnesium stearate.
Wrapper: talc, hypromellose, azorubin dye (E 122), simethicone emulsion 30% (dry mass), polyacrylate dispersion 30% (dry mass), titanium dioxide (E 171), macrogol 6000.

How to take, the dosage
The dose of the drug Mesim® forte is determined individually depending on the degree of digestive disorders.
Adults: 1 to 2 tablets before meals orally, without chewing and with water (the effectiveness of Mesim® forte may decrease if chewed, and pancreatic enzymes may irritate the oral mucosa when released).
If necessary, take 2 to 4 more tablets with meals.
If higher doses of pancreatin are required, the physician should be consulted to prescribe a higher dose depending on the symptoms (e.g., reduction in severity of steatorrhea, abdominal pain).
The duration of treatment may vary from a few days (for digestive problems caused by poor diet) to several months or even years (if permanent replacement therapy is necessary).
In children the drug is used by prescription.
Interaction
Absorption of folic acid may be decreased, which may require additional administration when concomitant use with pancreatin. The effect of hypoglycemic agents for oral administration (acarbose, miglitol) may be reduced when concomitant use with drugs containing digestive enzymes that break down carbohydrates (e.g., amylase).
Special Instructions
The drug Mesim® forte contains active enzymes which, if released in the oral cavity, e.g. when chewed, may damage the oral mucosa (up to and including ulceration). This is why the tablet must be swallowed whole.
Influence on the ability to drive vehicles and other mechanisms
The drug Mesim® forte has no effect or a slight effect on the ability to drive vehicles and mechanisms.
Synopsis
pink, flattened, coated tablets with nearly parallel surfaces and beveled edges, with a characteristic odor.
Contraindications
- high sensitivity to any of the ingredients of the drug.
- acute pancreatitis or exacerbation of chronic pancreatitis. However, it is possible to use in the phase of waning exacerbation when the diet is expanded, if there are signs of digestive disorders;
- children under 3 years of age (for this dosage form).

Immune system disorders

Very rare: gastrointestinal tract hypersensitivity (e.g., diarrhea, epigastric discomfort, nausea).
Overdose
No cases of overdose have been reported.
Pregnancy use
There are no data on the use of pancreatin during pregnancy and breastfeeding. During pregnancy and breastfeeding, pancreatin may be administered by the attending physician only if the expected benefits to the mother exceed the possible risk to the fetus or child.
