An antithyroid agent. It blocks the enzyme peroxidase involved in iodination of thyroid hormones of the thyroid gland, which leads to disruption of thyroxine and triiodothyronine synthesis.
Pharmacokinetics
After oral administration is well absorbed from the GI tract. T1/2 is 6 h. It is biotransformed in the body.
It is excreted in the urine as metabolites (70% within 48 hours) and unchanged.

How to take, the dosage
It is established individually. The usual dose is 20-40 mg/day in 2-4 doses, depending on the severity of the disease. After normalization of thyroid function (usually after 3-8 weeks), maintenance doses of 2.5-10 mg/day are used.
In children – 300-500 mcg/kg, maintenance doses – 200-300 mcg/kg. In maintenance doses, thiamazole can be taken once a day (in the morning after a meal).
Interaction
Concomitant use of thiamazole with amidopyrine, sulfonamides increases the risk of leukopenia.
Leukogen and folic acid when used concomitantly with thiamazole decrease the risk of leukopenia.
Gentamicin increases the antithyroid effect of thiamazole.
Special Instructions
The blood count should be monitored regularly during treatment.
Large thyroid strings with tracheal constriction should be treated with thiamazole for short periods of time, as prolonged treatment may cause the strings to grow.
Before starting treatment, patients should be warned to always see a doctor if they have fever, sore throat, inflammation of the mucous membrane of the mouth, or boils, because these manifestations can be symptoms of agranulocytosis.
1-2 weeks before surgery, the thiamazole should be stopped and the preparation for the strumectomy should be continued with iodine-containing medications.
Please note that iodine deficiency increases and excess weakens the effect of thiamazole.
Contraindications
Side effects
Allergic reactions: skin rash; rarely – fever; in single cases – lupus-like syndrome.
Hematopoietic system: rarely – disorders of hematopoiesis (leukopenia, agranulocytosis); in single cases – generalized lymphadenopathy.
Digestive system disorders: rare – taste disorders; in single cases – liver dysfunction, nausea, vomiting,
Nervous system and peripheral nervous system disorders: in single cases – neuritis and polyneuritis, headaches.
Endocrine system: thyroid hyperplasia.
Pregnancy use
It is contraindicated during pregnancy and lactation (breastfeeding).
Thiamazole penetrates the placental barrier. Use after the first trimester of pregnancy is associated with a risk of hypothyroidism in the fetus.
It is excreted with the breast milk.
