How to take, the dosage
Mercaptopurine is taken orally.
In adults and children, the initial oral dose is 2.5 mg/kg/day or 80-100 mg/m2 (average, 100-200 mg in adults, 50 mg in children 5 years of age).
The dose may be increased to 5 mg/kg/day (no more) for 4 weeks.
The maintenance dose: 1.5-2.5 mg/kg or 50-100 mg/m2 per day. In patients with hepatic and/or renal insufficiency the dose is reduced.
When prescribed concomitantly with allopurinol (300-600 mg/day), only 1/3-1/4 of the usual dose of mercaptopurine is used (reduction of metabolic intensity due to blockade of xanthine oxidase).
The daily dose of Mercaptopurine is usually used once.
It is also used in combination chemotherapy.
Contraindications
Hypersensitivity, tumor resistance, suppression of bone marrow function (leukopenia with a leukocyte count less than 3-109/l, thrombocytopenia with a platelet count less than 100-109/l), chickenpox, shingles and other infections, liver and renal dysfunction, history of gout or kidney stone disease.
Side effects
Nervous system and sense organs: headache, weakness.
Cardiovascular system and blood: most frequently – anemia, leukopenia, thrombocytopenia (during active therapy in acute myelogenous leukemia patients often have a period of relative bone marrow aplasia, with the importance of appropriate supportive treatment).
Gastrointestinal system disorders: nausea, vomiting, diarrhea, loss of appetite, hepatotoxic effect (has toxic-allergic genesis and occurs more frequently when the dose of 2.5 mg/kg/day is exceeded) or cholestasis (jaundice of sclerae or skin); rarely – stomatitis (usually in high doses), ulcerations in gastrointestinal tract, hepatic encephalopathy.
Urogenital system disorders: less frequently – hyperuricemia or nephropathy.
Skin disorders: skin hyperpigmentation, skin rash, itching.
Others: decreased immunity and predisposition to infections.
