Marcaine Spinal Heavy has a local anesthetic effect.
In intrathecal administration the effect is rapid and its duration varies from medium to long. The duration of the effect depends on the dose.
Bupivacaine reversibly inhibits impulse conduction along the nerve fiber by blocking the passage of sodium ions through the cell membrane.
Indications
Caudal, epidural, lumbar and conduction (in dentistry), retrobulbar anesthesia, sympathetic blockade.
Pharmacological effect

Antidepressants increase the risk of low blood pressure.

Contraindications
Hypersensitivity to any of the components of the drug or to other amide-type local anesthetics.
Marcain Spinal Heavy is contraindicated for IV anesthesia according to Beer.
Side Effects
symptoms of excitation or depression of the central nervous system (restlessness, anxiety, dizziness, tinnitus, blurred vision, tremors of the limbs, generalized convulsions),
limb paralysis,
paralysis of the respiratory muscles,
bradycardia,
dysfunction of the sphincters,
urinary retention,
paresthesia and numbness of the limbs,
impotence,
decreased cardiac output,
sinus node blockade
hypotension,
ventricular arrhythmias, including ventricular tachycardia,
heart failure.
Interaction
Use with MAO inhibitors (furazolidone, procarbazine, selegeline) increases the risk of excessive reduction in blood pressure. Vasoconstrictors (epinephrine, methoxamine, phenylephrine) prolong the local anesthetic effect of low-concentrated (0.125-0.25%) solutions of bupivacaine.
Local anesthetics enhance CNS depression caused by drugs that depress the CNS. Anticoagulants (ardeparin, dalteparin, danaparoid, enoxaparin, heparin, warfarin) increase the risk of bleeding. P
When treating the local anesthetic injection site with disinfectant solutions containing heavy metals, the risk of developing a local reaction in the form of pain and swelling increases.
When using local anesthetic drugs for spinal and epidural anesthesia with guanadrel, guanethidine, mecamylamine, trimethaphan, the risk of a pronounced decrease in blood pressure and bradycardia increases. Combination with general inhalation anesthesia with halothane increases the risk of bupivacaine cardiotoxicity. Strengthens and prolongs the effect of muscle relaxants. With narcotic analgesics, an additive effect develops, which is used during epidural anesthesia, but respiratory depression increases.
It exhibits antagonism with antimyasthenic drugs in its effect on skeletal muscles, especially when used in high doses, which requires additional correction in the treatment of myasthenia gravis. Cholinesterase inhibitors (antimyasthenic drugs, cyclophosphamide, demecarine, ecothiophate, isoflurofat, thiotepa) reduce the metabolism of local anesthetic drugs.
