Mannitol is an osmotic diuretic.

By increasing plasma osmotic pressure and filtration without subsequent reabsorption leads to water retention in the tubules and increased urine volume.

Elevating blood plasma osmolarity causes movement of fluid from the tissues (in particular the eyeball, brain) into the vascular bed. It does not affect glomerular filtration.

Diuresis is accompanied by a moderate increase in natriuresis without significant effect on excretion of potassium ions.

The diuretic effect is the greater the concentration (dose).

It is ineffective in impaired renal filtration function, as well as in azotemia in patients with cirrhosis and ascites.

Causes an increase in the BOD.

Indications

cerebral edema;
intracranial hypertension (with renal or renal-hepatic failure);
oliguria in acute renal or renal-liver failure with preserved filtration capacity of the kidneys (as part of combination therapy);
post-transfusion complications after the introduction of incompatible blood;
forced diuresis in case of poisoning with barbiturates and salicylates;
for the prevention of hemolysis during operations with extracorporeal circulation in order to prevent renal ischemia and associated acute renal failure.

By increasing the osmolarity of blood plasma, it causes the movement of fluid from tissues (in particular the eyeball, brain) into the vascular bed. Does not affect glomerular filtration.

The higher the concentration (dose), the higher the diuretic effect.

Ineffective in cases of impaired renal filtration function, as well as in azotemia in patients with liver cirrhosis and ascites.

Causes an increase in BCC.

Special instructions

In case of left ventricular failure (due to the risk of developing pulmonary edema), it is necessary to combine Mannitol with fast-acting loop diuretics. Monitoring of blood pressure, diuresis, and concentration of electrolytes in the blood serum (sodium and potassium ions) is necessary.

If headache, vomiting, dizziness, or visual disturbances occur during administration of the drug, the administration should be stopped and the development of complications such as subdural and subarachnoid bleeding should be excluded.

Possible use in heart failure (only in combination with loop diuretics) and hypertensive crisis with encephalopathy. Repeated administration of Mannitol should be carried out under the control of blood water and electrolyte balance.

There may be cases of crystallization of the drug during storage (at temperatures below 20 ° C). In case of crystallization, the bottle is heated to 50–70 °C until the precipitate dissolves. Use after cooling to body temperature if the crystals do not fall out again.

Contraindications

hypersensitivity to the components included in the drug;
anuria due to acute tubular necrosis in severe kidney damage;
severe dehydration;
left ventricular failure (especially accompanied by pulmonary edema);
chronic heart failure;
hemorrhagic stroke;
subarachnoid hemorrhage (except bleeding during craniotomy);
hyponatremia;
hypochloremia;
hypokalemia.

With caution: pregnancy, lactation, old age.

Side Effects

Dehydration (dry skin, dyspepsia, myasthenia gravis, convulsions, dry mouth, thirst, hallucinations, decreased blood pressure), impaired water and electrolyte metabolism (increased blood volume, hyponatremia; rarely, hyperkalemia).

Rarely – tachycardia, chest pain, thrombophlebitis, skin rash.

Interaction

Possible increased toxic effects of cardiac glycosides (associated with hypokalemia).
