Pharmgroup:

Antidiarrheal.

Pharmic action:

Loperamide, by binding to intestinal wall opioid receptors (stimulation of choline and adrenergic neurons via guanine nucleotides), reduces intestinal smooth muscle tone and motility (by inhibiting acetylcholine and PG release).

Delays peristalsis and increases the time of passage of intestinal contents. It increases tone of anal sphincter, promotes retention of feces and reduces urges to defecation. The action develops quickly and lasts for 4-6 hours.

Indications

Diarrhea (acute and chronic of different genesis: allergic, emotional, medicinal, radiation; with changes in diet and quality composition of food, with metabolic and absorption disorders).

Stool regulation in patients with ileostoma.

As an adjunctive drug – diarrhea of infectious genesis.

How to take, the dosage

Ingestion (capsules – without chewing, with water; lingual tablet – on the tongue, within a few seconds it disintegrates, after which it is swallowed with saliva, without water).

In acute diarrhea, adults are prescribed an initial dose of 4 mg; then 2 mg after each act of defecation (in case of liquid feces); the highest daily dose is 16 mg.

When administered in drops: the initial dose is 60 drops of 0.002% solution; then 30 drops after each act of defecation; the maximum dose is 180 drops/day (6 times).

In chronic diarrhea, adults are prescribed 4 mg/day. The maximum daily dose is 16 mg.

In acute diarrhea, children over 5 years of age are prescribed an initial dose of 2 mg, then 2 mg after each act of defecation; the maximum daily dose is 8 mg.

Drops: the initial dose is 30 drops of 0.002% solution; then 30 drops 3 times daily; the maximum dose is 120 drops/day (4 doses).

In chronic diarrhea, loperamide is indicated in a daily dose of 30 drops or 2 mg for children over 5 years of age.

In children aged 2-5 years old it is indicated in solution for oral administration in dose of 5 ml (1 capful) per 10 kg; the frequency of administration is 2-3 times per day.

The maximum daily dose is 6 mg per 20 kg. In case of normal stools or in absence of stools for more than 12 hours the drug should be discontinued.

Interaction

Opioid analgesics increase the risk of severe constipation.

Special Instructions

If there is no effect after 2 days of using Loperamide, a doctor should be consulted.

If constipation or bloating develop during treatment, loperamide should be discontinued. In patients with impaired liver function, close monitoring of signs of toxic CNS damage is necessary.

The loss of fluids and electrolytes must be replenished during treatment of diarrhea.

During treatment, caution should be exercised while driving vehicles and engaging in other potentially hazardous activities that require increased concentration and quick psychomotor reactions.

Contraindications

With caution:hepatic failure.

Side effects

Allergic reactions (skin rash), drowsiness, dizziness, hypovolemia, electrolyte disturbances;

Dry oral mucosa, colic, gastralgia, pain or discomfort in the stomach, nausea, vomiting, constipation, flatulence.

Rarely – urinary retention, extremely rare – intestinal obstruction.

When taking oral tablets, a burning or tingling sensation on the tongue.

Overdose

Symptoms:

CNS depression (stupor, coordination disorders, drowsiness, miosis, muscle hypertonia, respiratory depression), intestinal obstruction.

Treatment:

The antidote is naloxone; given that the duration of action of loperamide is longer than that of naloxone, reintroduction of the latter is possible. Symptomatic treatment, activated charcoal, gastric lavage, ventilator.
