Pharmic action:
Locoid Lipocrem is a synthetic nonhalogenated glucocorticoid drug for topical use. The esterification of the hydrocortisone molecule with an oily acid residue (butyrate) allowed to radically increase the activity of the drug compared to the native hydrocortisone. It has a rapid onset anti-inflammatory, anti-edematous, antipruritic effect.
The administration of Locoid Lipocrem® in recommended doses does not cause suppression of the hypothalamic-pituitary-adrenal system. Although use of the drug in high doses over a long period of time, especially with occlusive dressings, may increase plasma cortisol levels, this is not usually accompanied by a decrease in pituitary-adrenal reactivity, and withdrawal of the drug results in a rapid normalization of cortisol production.

Absorption
After application, the active substance accumulates in the epidermis, mainly in the granular layer. Systemic absorption is insignificant. A small amount of hydrocortisone 17-butyrate is absorbed into the systemic bloodstream unchanged.
Metabolism
Most hydrocortisone 17-butyrate is metabolized to hydrocortisone and other metabolites directly in epidermis and subsequently in liver.
Elimation
Metabolites and a small portion of unchanged hydrocortisone 17-butyrate are excreted with the kidneys and through the intestine.
Indications
Superficial, non-infected skin diseases sensitive to local corticosteroids:
– eczema;
— dermatitis (including atopic, contact, seborrheic);
– psoriasis.

Special instructions
Avoid contact of the ointment with the eyes.
For children under 12 years of age, the drug is prescribed only under strict medical supervision. If after 7 days of use there is no improvement or worsening of the condition, as well as if symptoms recur a few days after discontinuation, use of the drug should be discontinued and consult a doctor.
The drug should not be used without consulting a doctor in cases where itching in the genital area in women is accompanied by vaginal discharge.
For long-term treatment and/or application to large surfaces, it is recommended to prescribe a diet with limited Na+ and a high content of K+; introduce a sufficient amount of protein into the body. It is necessary to monitor blood pressure, the concentration of glucose and cortisol in the blood, blood clotting, diuresis, and the patient’s body weight.

1 g contains hydrocortisone 17-butyrate 1 mg,
excipients:
liquid paraffin – 180 mg,
white soft paraffin – 420 mg,
cetostearyl alcohol – 60 mg,
cetomacrogol 25 – 30 mg,
propyl parahydroxybenzoate – 0.5 mg,
benzyl alcohol – 5 mg,
anhydrous citric acid – 1.8 mg,
sodium citrate anhydrous – 1.2 mg,
purified water – 300.5 mg.
Contraindications
– bacterial skin infections (strepto- and staphyloderma, gram-negative folliculitis);
— viral skin infections (herpes simplex, chicken pox, shingles);
– fungal skin infections;
– tuberculous and syphilitic skin lesions;
– parasitic skin infections;
— neoplastic skin changes (benign and malignant tumors);
— acne, rosacea, perioral dermatitis;
— post-vaccination period;
— violation of the integrity of the skin (wounds, ulcers);
— hypersensitivity to the drug Lokoid Lipokrem.
The drug should be prescribed with caution during pregnancy and lactation.
Side Effects
Local reactions: skin irritation; in rare cases, other local reactions characteristic of GCS therapy.
Systemic reactions: with long-term use, application to large surfaces and/or use of occlusive dressings, side effects characteristic of systemic corticosteroids may be observed.
The risk of local and systemic side effects when using Lokoid Lipokrem® is lower than when using fluorinated steroids.
Storage conditions
The drug should be stored out of the reach of children at a temperature not exceeding 25°C.
