Pharmic action:
Lokoid Crelo® is a synthetic nonhalogenated glucocorticosteroid drug for topical use. The esterification of the hydrocortisone molecule with an oily acid residue (butyrate) has radically increased the activity of the drug compared to native hydrocortisone. Locoid Crelo® has a fast-onset anti-inflammatory, anti-edematous, antipruritic effect.
The use of Locoid Crelo® in recommended doses does not cause suppression of the hypothalamic-pituitary-adrenal system. Although long-term use of high doses of the drug, especially with occlusive dressings, may increase plasma cortisol levels, this is not usually accompanied by a decrease in pituitary-adrenal reactivity, and withdrawal of the drug leads to rapid normalization of cortisol production.
Pharmacokinetics:
Absorption
After application, the active substance accumulates in the epidermis, mainly in the granular layer, with little systemic absorption. A small amount of hydrocortisone 17-butyrate is absorbed into the systemic bloodstream unchanged.
Metabolism
Most hydrocortisone 17-butyrate is metabolized to hydrocortisone and other metabolites directly in the epidermis and subsequently in the liver.
Elimination
The metabolites and a small portion of unchanged hydrocortisone 17-butyrate are excreted by the kidneys and through the intestine.
Indications
Superficial, non-infectious, sensitive to local GCS skin diseases:
– eczema;
– dermatitis (including atopic, contact, seborrheic);
– psoriasis.

How to take, the dosage
Locoid Crelo is used topically.
The affected skin surface is smeared thinly no more than 3-4 times a day.
The course of treatment is 1-3 weeks.
The dose of the drug used for 1 week should not exceed 30-60 g.
In case of itching in the anus area, before applying the drug, the affected area should be washed with warm water and soap, then water and dry the skin surface with a towel or tissue.
For blistering skin conditions, lesions on the scalp use lotion and emulsion for external use.
Special Instructions
Eye contact with the ointment should be avoided. Children under 12 years of age are prescribed the drug only under strict medical supervision.
If after 7 days of use there is no improvement or deterioration, or if symptoms return several days after discontinuation, the drug should be stopped and a physician should be consulted.
The drug should not be used without consulting a physician in cases where itching in the genital area in women is accompanied by vaginal discharge.
If prolonged treatment and/or application to large surfaces, it is recommended to prescribe a diet with restriction of Na+ and increased K+; to introduce an adequate amount of protein into the body.
The patient’s BP, blood glucose and cortisol concentrations, blood clotting, diuresis, and body weight should be monitored.
Contraindications
– bacterial skin infections (strepto- and staphylodermia, gram-negative folliculitis);
– viral skin infections (herpes simplex, varicella, herpes zoster);
– fungal skin infections;
– tuberculosis and syphilitic skin lesions;
– parasitic skin infections;
– neoplastic skin changes (benign and malignant tumors);
– acne, rosacea, perioral dermatitis;
– postvaccination period;
– skin integrity disorders (wounds, ulcers);
– hypersensitivity to Locoid cream.
The drug should be administered with caution during pregnancy and lactation.
Side effects
Local reactions: skin irritation; in rare cases – other local reactions typical for GCS therapy.
Systemic reactions: with long-term use, application on large surfaces and/or use of occlusive dressings the side effects typical for systemic GCS may be observed.
The risk of local and systemic side effects with Locoid Crelo® is lower than with fluorinated steroids.
Overdose
There are no data on overdose of Locoid Crelo®.
Long-term use of the drug in high doses may increase the above side effects.
The drug should be discontinued if symptoms of hypercorticism occur.
