Pharmacological properties
Lactobacillus casei, along with other species of lactobacilli (L. Asidorhilus, L. fermentum, L. cellobiosum, etc.) are present in the normal flora of the vulva and vagina of women. Colonization of the vagina with live L. casei rhamnosus Doderleini bacilli creates a physiological-like environment in the vagina. Strain L. casei rhamnosus Doderleini in the preparation Lactoginal ® has antagonistic activity against a wide spectrum of pathogenic and opportunistic bacteria (including Staphylococcus spp., Proteus spp., enteropathogenic Escherichia coli), improves local metabolic processes and helps restore the natural local immunity.
When the capsule of the drug Lactoginal ® is inserted into the vagina, the gelatinous shell of the capsule dissolves with the release of lyophilized culture of live lactobacilli. In vitro, L. casei rhamnosus Doderleini has been shown to lower the pH for several hours. This property of L. casei rhamnosus Doderleini when injected into the vagina helps to restore its natural microflora.
Lactobacillus deficiency is an important factor in the development of candidiasis vulvovaginitis. Lactobacilli are physiological antagonists of yeast-like fungi of the genus Candida through the secretion of antifungal substances and competition for receptors on vaginal epitheliocytes (they inhibit Candida attachment to vaginal epitheliocytes and inhibit their growth and reproduction).
The results of clinical trial have shown effectiveness of Lactoginal® in restoration of normal vaginal microflora and prevention of recurrent candidal vulvovaginitis when used after completion of therapy with antifungal agents.
In an in vitro study, Lactobacillus casei subsp. rhamnosus LCR35 was shown to be resistant when co-cultured with a number of antibacterial drugs, including metronidazole and clindamycin. In addition, in vitro and in vivo studies revealed the phenomenon of potentiation of antibiotic action by metabolites of probiotic strain Lactobacillus casei subsp. rhamnosus LCR35, which may improve the effectiveness of the therapy of bacterial vaginosis when the antibiotic and Lactoginal® are used simultaneously The results of the clinical study showed the effectiveness of Lactoginal® in the therapy of bacterial vaginosis when used simultaneously with the antibacterial drug clindamycin, vaginal cream 2%.
Indications
Restoration of normal vaginal microflora in bacterial vaginosis from the first day of antibiotic therapy or immediately after its completion; in preparation for elective gynecological operations; in prenatal preparation of pregnant women at risk for bacterial vaginosis.
The prevention of recurrent vulvovaginal candidiasis (including exacerbations of recurrent vulvovaginal candidiasis) after topical and/or systemic antifungal therapy.

Active ingredient:
Lyophilized culture of L. casei rhamnosus Doderleini not less than 1×108 COE viable bacteria.
Auxiliary substances: magnesium stearate 3.41 mg, lactose monohydrate – 5.59 mg, components of the protective drying medium (potato starch, lactose monohydrate, sodium glutamate, sodium thiosulfate).
Wrapper: gelatin.

How to take, the dosage
The method of application is intravaginal.
Inject the capsule deep into the vagina, moistening it with a little water to speed up dissolution of the drug.
The restoration of normal vaginal microflora in the therapy of bacterial vaginosis:
In preparation for elective gynecologic surgeries, prenatal preparation of pregnant women at risk for bacterial vaginosis: one capsule daily in the morning and in the evening for 7 days or one capsule daily for 14 days – as part of complex therapy.
The dosing regimen should be adjusted according to the severity of the clinical effect.
Prevention of recurrent vulvovaginal candidiasis (including exacerbation of recurrent vulvovaginal candidiasis) after local and/or systemic therapy with antifungal agents: 1 capsule at night. The duration of therapy with the drug shall be determined by the physician. The course of treatment shall not exceed 21 days. The question of repeated courses is decided individually.
If after treatment, no improvement or worsening of symptoms or appearance of new symptoms occurs, it is necessary to consult a physician. Use the drug only according to the indication, route of administration, and dosage listed in the directions for use.
Interaction
The use of the drug together with spermicides is not recommended.
Possible simultaneous use with antibacterial, antiviral and immunomodulatory drugs.
Clinically significant cases of interaction have not been registered.
