Heparin ointment 25 g

A combination drug for external use, the effect of which is due to the properties of its constituent components.

Heparin is a direct-acting anticoagulant and belongs to the group of medium molecular weight heparins.

In external use, it has a local anticoagulant, anti-exudative, moderate anti-inflammatory effect.

Blocks the formation of thrombin. Heparin reduces inflammation and has anticoagulant effect, indirectly improves microcirculation, which contributes to the process of resorption of hematomas and clots and reduction of tissue swelling.

Benzylnicotinate is a peripheral vasodilator and improves microcirculation. It is used as an adjuvant to improve heparin penetration.

Benzocaine has a local anesthetic effect.

Limits the permeability of the cell membrane to sodium ions, displaces calcium ions from the receptors located on the inner surface of the membrane, blocks the conduction of nerve impulses.

Inhibits the occurrence of pain impulses in the endings of sensitive nerves and their conduction along the nerve fibers.

Indications

prevention and treatment of superficial vein thrombophlebitis;
post-injection and post-infusion phlebitis;
external hemorrhoids;
inflammation of postpartum hemorrhoids;
trophic ulcers of the leg;
elephantiasis;
superficial periphlebitis;
lymphangitis;
superficial mastitis;
localized infiltrates and edema;
injuries and bruises without violating the integrity of the skin (including muscle tissue, tendons, joints);
subcutaneous hematoma.

Special instructions

With prolonged use of Heparin ointment over large areas and simultaneous use of oral medications that affect the blood coagulation system and/or hematopoiesis, clotting time and prothrombin time should be monitored.

If a long course of treatment is necessary and for large areas of damage in pregnant women and women over 65 years of age, similar monitoring is recommended.

Should not be applied to open wounds in the presence of purulent processes.

The use of ointment is not recommended for deep venous thrombosis.

1 g ointment contains:

Heparin sodium – 100 IU (0.833 mg with activity of heparin sodium 120 IU/mg),

Benzocaine (anesthesin) – 40.0 mg,

Benzylnicotinate – 0.8 mg;

Excipients:

Glycerol – 150 mg,

Vaseline – 60 mg,

Stearic acid (stearic acid 50) – 50 mg,

Peach oil – 50 mg,

Emulsifier No. 1 – 50 mg,

Methyl parahydroxybenzoate – 1.5 mg,

Propyl parahydroxybenzoate – 0.5 mg,

Water – up to 1 g.

Contraindications

Hypersensitivity to the components of the drug; violation of the integrity of the skin of the affected area (open infected wounds, ulcerative-necrotic processes in particular); children under 1 year of age.

With caution

Blood coagulation disorders, thrombocytopenia.

Side Effects

With prolonged use of the drug, local reactions are possible in the form of skin hyperemia, decreased sensitivity at the site of application, allergic reactions (skin rash, itching).

Clinical pharmacology

Heparin is slightly absorbed from the surface of the skin. Bonding with plasma proteins is up to 95%, volume of distribution is 0.06 l/kg. Does not cross the placenta or into breast milk.

Intensively captured by endothelial cells and cells of the mononuclear-macrophage system, concentrated in the liver and spleen.

Metabolized in the liver with the participation of N-desulfamidase and platelet heparinase, which is involved in the metabolism of heparin at later stages.

Desulfated molecules are converted into low molecular weight fragments by kidney endoglycosidase.

Half-life (T1/2) – 1-6 hours (average – 1.5 hours); increases with obesity, liver and/or kidney failure; decreases with pulmonary embolism, infections, malignant tumors.
It is excreted by the kidneys, mainly in the form of inactive metabolites. Not excreted by hemodialysis.

Benzyl nicotinate penetrates into all layers of the skin. It practically does not enter the systemic circulation.

Benzocaine is practically not absorbed when applied externally.
