Beta-adrenergic stimulating agent, with selectivity towards the beta2-adrenoreceptors of bronchial smooth muscle, has a bronchodilator effect.
Foradil is effective for the prevention and relief of bronchospasm caused by allergens, physical activity, histamine, methacholine (diagnostic test). Foradil has tocolytic properties.
The onset of action is 1-3 minutes after inhalation, the duration of bronchodilator effect is 12 hours.
Indications
Coping and prevention of reversible airway obstruction – COPD, bronchial asthma (including atopy and exercise asthma).
Active ingredient
Composition
1 capsule contains formoterol fumarate 12 mcg.
How to take, the dosage
Foradil is used by inhalation.
Adults with bronchospastic syndrome – 12-24 mcg 2 times a day, children from 5 years and older – 12 mcg (no more) 2 times a day; if necessary, you can use an additional 1-2 capsules. To prevent bronchospasm caused by exercise or exposure to a known allergen, inhale 1 dose (12 mcg) about 15 minutes before work or exposure to that factor; in severe cases of bronchial asthma, 2 doses (24 mcg) may need to be inhaled.
Interaction
Drugs with adrenergic stimulating activity, MAO inhibitors, tricyclic antidepressants increase the risk of adverse cardiovascular effects.
Xanthine derivatives, GCS, diuretics increase the possibility of hypokalemic action of the drug.
Hinidine, disopyramide, procainamide, phenothiazines, antihistamines, tricyclic antidepressants increase the risk of ventricular arrhythmias. Beta-adrenoblockers (including in the form of eye drops) partially or completely block the action.
Special Instructions
Patients should be trained by their physician or medical staff on the use of the inhaler. Patients treated with GCS inhalation in combination with the aerosol should not discontinue GCS treatment.
Patients with diabetes mellitus should have their blood glucose concentrations monitored.
Patients with a severe course of bronchial asthma have an increased risk of hypokalemia with hypoxia and concomitant therapy; therefore, they require regular monitoring of serum K+.
The possibility of use in small children depends on the child’s individual ability to use the inhaler correctly (including breathing technique), use only under the supervision of an adult.
Tremors or anxiety that occur during treatment with beta-adrenergic stimulants may affect the patient’s ability to drive.
Contraindications
Hypersensitivity, children (under 5 years).
With caution.
Diabetes mellitus, severe CHD, CHD, cardiac rhythm disorders, AV blockade III, Q-T interval prolongation (Q-T corrected > 0.44 s), GOCMP, thyrotoxicosis, pregnancy, lactation period.
Side effects
Unwanted reactions are distributed according to the frequency of occurrence. The following criteria were used to assess the frequency: very common (â¥10%); common (â¥1%, Allergic reactions: very rare – hypersensitivity reactions such as hypotension, urticaria, angioedema, pruritus, exanthema.
CNS disorders: often – headache, tremor; sometimes – agitation, anxiety, nervousness, insomnia, dizziness.
The cardiovascular system: often – palpitations; sometimes – tachycardia; very rarely – peripheral edema.
Respiratory system: sometimes – bronchospasm.
The digestive system: very rare – nausea; distortion of taste.
Muscular system: sometimes – muscle cramps, myalgia.
Local reactions: sometimes – irritation of the mucous membranes of the pharynx and larynx.
Overdose
Symptoms: nausea, vomiting, headache, tremor, somnolence, ventricular arrhythmia, decreased BP, metabolic acidosis, hypokalemia, hyperglycemia.
