Bronchoobstructive syndrome in bronchial asthma, bronchitis, pulmonary emphysema, cardiac asthma (mainly for relief of attacks); hypertension in the small circle of the circulation.
Cerebral vascular crises of atherosclerotic origin and as part of combined therapy for ischemic strokes and chronic insufficiency of the cerebral circulation to reduce intracranial pressure and cerebral edema.
Indications
Broncho-obstructive syndrome in bronchial asthma, bronchitis, emphysema, cardiac asthma (mainly to relieve attacks); hypertension in the pulmonary circulation.

Special instructions
Aminophylline solutions are not injected under the skin, as they have a local irritating effect.

When treated with the drug, patients are not recommended to perform work for 24 hours that requires a quick reaction and is associated with risk, for example, driving vehicles.

Contraindications
Severe arterial hypotension or hypertension, paroxysmal tachycardia, extrasystole, myocardial infarction with cardiac arrhythmias, epilepsy, increased convulsive readiness, hypertrophic obstructive cardiomyopathy, thyrotoxicosis, pulmonary edema, severe coronary insufficiency, hepatic and/or renal failure, hemorrhagic stroke, retinal hemorrhage, recent history of bleeding, hypersensitivity to the drug.
With caution:
Pregnancy, neonatal period, age over 55 years and uncontrolled hypothyroidism (possibility of cumulation), sepsis, prolonged hyperthermia, gastroesophageal reflux, prostate adenoma.
The drug is prescribed with caution to patients with severe impairment of liver and kidney function, gastric ulcer and duodenal ulcer.
Side Effects
From the digestive system: gastroesophageal reflux, gastralgia, nausea, vomiting, heartburn, exacerbation of peptic ulcer.
From the nervous system: headache, insomnia, agitation, anxiety, restlessness, irritability, dizziness, tremor.
From the cardiovascular system: palpitations, arrhythmias, tachycardia, heart rhythm disturbances, decreased blood pressure, cardialgia, increased frequency of angina attacks.
Allergic reactions: skin rash, skin itching, fever.
Local reactions: reactions at the injection site – hyperemia, pain.
Other: chest pain, tachypnea, albuminuria, hematuria, hypoglycemia, increased sweating, increased diuresis.
Interaction
Ephedrine, beta-agonists, caffeine and furosemide enhance the effect of the drug. In combination with phenobarbital, diphenin, rifampicin, isoniazid, carbamazepine or sulfinpyrazone, a decrease in the effectiveness of aminophylline is observed, which may require an increase in the doses of the drug used.
The clearance of the drug is reduced when prescribed in combination with macrolide antibiotics, lincomycin, allopurinol, cimetidine, isoprenaline, beta-blockers, which may require a dose reduction.
Oral estrogen-containing contraceptives, antidiarrheal drugs, intestinal sorbents weaken, and H2-histamine blockers, slow calcium channel blockers, mexiletine enhance the effect (they bind to the cytochrome P450 enzymatic system and slow down the metabolism of aminophylline).
When used in combination with epoxacin and other fluoroquinolines, the dose of aminophylline is reduced. The drug suppresses the therapeutic effects of lithium carbonate and beta-blockers.
The administration of beta-blockers interferes with the bronchodilatory effect of aminophylline and can cause bronchospasm. Eufillin potentiates the effect of diuretics by increasing glomerular filtration and reducing tubular reabsorption.
With caution, aminophylline is prescribed simultaneously with anticoagulants, other theophylline or purine derivatives. It is not recommended to use aminophylline with drugs that excite the central nervous system (increases neurotoxicity).
Eufillin solution for intramuscular administration cannot be diluted or mixed with other solutions.
Overdose
In case of overdose, anorexia, diarrhea, nausea, vomiting, pain in the epigastric region, gastrointestinal bleeding, tachycardia, ventricular arrhythmias, tremor, generalized convulsions, hyperventilation, and a sharp decrease in blood pressure are observed.
Treatment of overdose depends on the clinical picture and includes drug withdrawal, stimulation of its removal from the body (forced diuresis, hemosorption, plasma sorption, hemodialysis, peritoneal dialysis) and the use of symptomatic agents. Diazepam (by injection) is used to relieve seizures. Barbie turates should not be used.
In case of severe intoxication (eufillin content more than 50 g/l), hemodialysis is recommended.
