Pharmacological action – metabolic.
L-carnitine is a natural substance related to the B vitamins. It participates in metabolic processes as a carrier of long-chain fatty acids (palmitic acid and others) from the cytoplasm to the mitochondria where these acids undergo a process of β-oxidation to form adenosine triphosphoric acid and acetyl-CoA.
It improves protein and fat metabolism, increases the secretion and enzymatic activity of gastric and intestinal juices, improves digestion, reduces excess body weight and muscle fat content.
It increases resistance to physical activity, inhibits ketoacid formation and anaerobic glycolysis, reduces lactacidosis, promotes economical use of glycogen and increases its reserves in the liver and muscles.
It has anabolic and lipolytic action. It normalizes the increased basic metabolism in hyperthyroidism: it is a peripheral (indirect) antagonist of the thyroid hormones, rather than a direct inhibitor of thyroid function. It improves metabolism and tissue energy supply.
Indications
As part of the treatment:
– intense physical activity and psycho-emotional stress: to improve performance, endurance, reduce fatigue, including.Including elderly people;
– During rehabilitation after diseases and surgery, injuries.
– complex therapy of chronic gastritis with decreased secretory function and chronic pancreatitis with external secretory insufficiency;
– complex therapy of skin diseases (psoriasis, seborrheic eczema, focal scleroderma, lupus erythematosus);
– Hyperthyroidism of light degree;
– Neurological manifestations in brain injuries of vascular, toxic and traumatic genesis;

In pediatric practice (treatment of children under 3 years old under medical supervision):

Active ingredient
Composition
1 ml of oral solution contains: levocarnitine (carnificite) 300 mg.
Auxiliary substances:
citric acid monohydrate 1.2 mg,
methylparahydroxybenzoate 0.5 mg,
propylparahydroxybenzoate 0.2 mg,
purified water up to 1 ml.
How to take, the dosage
The drug is used as part of the complex therapy. Orally, 30 minutes before the meal, additionally diluted with liquid.
In case of long physical and psycho-emotional strain: from 0.75 g (1/2 scoop or 2.5 ml) 3 times a day to 2.25 g (1.5 scoops or 7.5 ml) 2-3 times a day.
In case of anorexia nervosa and also during rehabilitation after diseases and surgical interventions and injuries: 1.5 g (1 scoop or 5 ml) 2 times a day. The course of treatment is 1-2 months.
In the complex treatment of chronic gastritis and chronic pancreatitis with decreased secretory function: 0.375 g (1/4 scoop or 1.25 ml) 2 times a day. The course of treatment is 1-1.5 months.
For treatment of skin diseases: 0.75 g (1/2 scoop or 2.5 ml). The course of treatment is for 2-4 weeks.
In mild hyperthyroidism: 0.25 g (13 drops) 2-3 times a day. The course of treatment is 20 days. The course of treatment is repeated after 1-2 months’ break or it is prescribed for 3 months without break.
In case of vascular, toxic and traumatic brain injury it is taken in 0.75 g (1/2 scoop or 2.5 ml) a day. The course of treatment is 3-5 days. If necessary the second course of treatment is prescribed in 12-14 days.
In case of carnitine deficiency diseases (primary and secondary carnitine deficiency): up to 50-100 mg/kg (2-5 drops/kg) of body weight with a multiple use of 2-3 times a day. The course of treatment is 3-4 months.
Children are prescribed in the form of additive to sweet meals (kissel, compote, juices). The dose for children under 3 years old is determined by the attending physician. From 3 to 6 years old – a single dose of 0.1 g (5 drops) 2-3 times a day, the daily dose – 0.2-0.3 g (11-16 drops). Course of treatment – 1 month, children 6 to 12 years old are prescribed a single dose of 0.2-0.3 g (11-16 drops) 2-3 times a day, a daily dose of 0.4-0.9 g (22-48 drops). The course of treatment is at least 1 month.
In case of growth retardation: 0.25 g (13 drops) 2-3 times a day. The course of treatment is 20 days. The course of treatment is repeated after 1-2 months break or administered for 3 months without a break.
In sports medicine and intensive training: 2.5 g 1-3 times a day (daily dose of 2.5 -7.5 g). If used therapeutically 70-100 mg/kg/day (5-7,5 g/day). The course of treatment: 3-4 weeks in the pre-competition period. During the training process up to 6-8 weeks.
Interaction
Glucocorticosteroids promote accumulation of the drug in tissues (except the liver), other anabolics enhance the effect.
Special Instructions
Treatment of children under the age of 3 years should be under the supervision of a physician.
Impact on the ability to drive and operate machinery:
The drug has no effect on the performance of potentially hazardous activities requiring special attention and quick reactions (driving and other vehicles, working with moving machinery, work of a dispatcher and operator, etc.etc.)
Contraindications
Individual intolerance to the ingredients of the drug.
Side effects
Possible: allergic reactions, gastralgia, dyspepsia, myasthenia (in patients with uremia).
Overdose
Symptoms: dyspeptic disorders, myasthenic disorders (in patients with uremia).
Treatment: gastric lavage, taking activated charcoal.
Pregnancy use
Pregnancy is contraindicated due to a lack of studies.
Breastfeeding should be stopped while using the drug.
