Ednit is an ACE inhibitor. Antihypertensive drug.
Enalapril maleate is a prodrug: as a result of its hydrolysis enalaprilate is formed, which inhibits ACE activity, thus inhibiting angiotensin II formation.
The drug decreases OPPS, increases cardiac output, decreases end-diastolic pressure in left ventricle, decreases pressure in pulmonary veins and arteries, right atrium.
Long-term use of the drug reduces severity of myocardial hypertrophy of the left ventricle, prevents progression of heart failure and slows development of left ventricular dilatation.
It has some diuretic effect. Reduces intraclavian hypertension, slowing the development of glomerulosclerosis and the risk of renal failure.
Indications
– Arterial hypertension (including renovascular), both as monotherapy and in combination with other antihypertensive agents.
– Chronic heart failure (as part of combined therapy).

Composition
1 tablet contains:
enalapril maleate 20 mg.
How to take, the dosage
In treatment of essential arterial hypertension, the initial daily dose of the drug for adults is 10-20 mg. Further the dose is adjusted individually.
In moderate hypertension the average daily dose is 10 mg. The maximum daily dose is 40 mg.
In renovascular hypertension, the initial daily dose is 2.5-5 mg. The maximum daily dose is 20 mg.
In chronic heart failure, the initial daily dose of Ednit is 2.5 mg, then the dose is gradually increased to 10-20 mg (in 1 or 2 doses).
Patients on hemodialysis are prescribed Ednit at a dose of 2.5 mg/day. (but note that the drug penetrates through dialysis membranes).
The duration of treatment depends on the effectiveness of therapy. If the BP decrease is too pronounced, the dose of the drug is gradually reduced.
Interaction
When concomitant use of Ednit with NSAIDs may decrease the hypotensive effect.
Concomitant use of Ednit and potassium-saving diuretics or potassium preparations may result in hyperkalemia (blood potassium levels should be monitored when using this combination).
Ednit reduces the effect of drugs containing theophylline. Cimetidine prolongs the effect of Ednit. When concomitant use with ethanol an increase in the hypotensive effect of Ednit is noted.
In concomitant use of Ednit and lithium preparations slower excretion of lithium is noted.
Special Instructions
With caution, the drug should be administered in autoimmune diseases, liver dysfunction, fluid and salt loss, bilateral renal artery stenosis or stenosis of the artery of the single kidney, when concomitantly taken with immunosuppressants and diuretics.
In case of previous treatment with diuretics (for example, in patients with chronic heart failure) the risk of orthostatic hypotension increases, therefore it is necessary to compensate fluid and salt loss before starting treatment with Ednit.
Ednit should be discontinued before parathyroid function tests.
Patients taking Ednit should refrain from excessive alcohol consumption.
Contraindications
– Condition after kidney transplantation.
– Primary hyperaldosteronism.
– Pregnancy.
– Lactation (breastfeeding).
– Childhood.
– Hypersensitivity to enalapril or other ACE inhibitors.
– Information in the anamnesis about the development of angioedema while taking ACE inhibitors.
Side effects
Cardiovascular system: possibly – arterial hypotension (including orthostatic), collapse; rarely – palpitation, angina pectoris, pain in the heart area, heart rhythm disorders.
CNS and peripheral nervous system disorders: headache, dizziness, increased fatigability, somnolence (2-3%); in some cases when the drug is used in high doses – nervousness, depression, paresthesia, loss of balance, tinnitus, decreased visual acuity.
Digestive system disorders: dry mouth, nausea, vomiting, diarrhea, increased liver transaminase activity, increased concentration of bilirubin in blood; dyspepsia, pancreatitis; in single cases – change of taste.
Respiratory system: dry cough, shortness of breath, bronchitis, bronchospasm, pneumonia, hoarseness of voice.
Hematopoietic system disorders: neutropenia; in patients with autoimmune diseases in some cases – agranulocytosis.
Urinary system disorders: proteinuria.
Water-electrolyte balance: hyperkalemia.
Allergic reactions: rarely – rash, urticaria, itching, in some cases – angioedema.
Dermatological reactions: in some cases – photosensitization, hair loss.
Other: sexual disorders, hot flashes.
Overdose
Symptoms: arterial hypotension.
Treatment: the patient should be put in horizontal position with elevated legs.
In mild cases, saline solution is administered orally.
In more severe cases, intravenous sodium chloride isotonic solution or plasma substitutes.
Hemodialysis may be performed.
Pregnancy use
Ednit is contraindicated in pregnancy.
If it is necessary to use Ednit during lactation, discontinuation of breastfeeding should be considered.
