Dolobene Gel, 100 g

Pharmaceutical group:

A drug with anti-inflammatory, anti-exudative, analgesic, antithrombotic and tissue regeneration improving action for external use.

Pharmic action:

Dimethylsulfoxide (DMSO) has anti-inflammatory, anti-edema and local analgesic action.
Anti-inflammatory activity is associated with several pharmacological effects, the most significant of which is inactivation of hydroxyl radicals produced in large quantities during inflammation and having a damaging effect on tissues.

DMSO provides local analgesic effect due to reduction of conduction speed of nociceptive (pain) impulses in peripheral neurons. Antiedematous effect is provided by inactivation of hydroxyl radicals and improvement of subcutaneous metabolic reaction in the place of application of the drug. To a certain extent, hygroscopic properties of DMSO are also responsible for its anti-edema effect. DMSO (50% and more) penetrates through biological membranes, including skin, facilitating better and deeper penetration into tissues of other drugs used simultaneously with it.

Heparin.

Heparin has anti-inflammatory effect, promotes regeneration of connective tissue by inhibiting the activity of hyaluronidase. Heparin shows dose-dependent antithrombotic effect, increasing the inhibitory activity of antithrombin III on prothrombin and thrombin activation. Heparin penetration through healthy skin is dose-dependent and is confirmed for doses starting at 300 IU/g.

Dexpanthenol.

Dexpanthenol is transformed in the skin into pantothenic acid (a B vitamin) when applied topically. The effectiveness of dexpanthenol is comparable to that of pantothenic acid. Pantothenic acid as a component of coenzyme A is involved in various catabolic and anabolic processes in tissues, by improving the processes of granulation and epithelization helps to regenerate damaged skin.

Indications

– swellings, bruises and inflammations of soft tissues, muscles, tendons, tendon sheaths,
– closed injuries, contusions,
– injuries of joints with ligament and tendon sprains,
– injuries of joints with ligament and tendon sprains. – epicondylitis of the shoulder (“tennis players’ elbow”), tendinitis (tendon inflammation), tendovaginitis (tendon sheath inflammation), bursitis (inflammation of mucous bag of the joint),
– periarthritis of shoulder joints,
– acute neuralgia.

Active ingredient

Composition

100 g:

– Sodium heparin 50000 ME>

– Dexpanthenol 2.500 g

– Dimethyl sulfoxide 90% 16.66 g (in terms of dry substance – 15 g)

Auxiliary substances:

Polyacrylic acid 1,000 – 1,400 g,

tromethamol 0,200 – 0,300 g,

macro-gluceryl hydroxystearate (Cremophor CO 455) 1.000 g,

35,000 g isopropanol,

Rosemary oil 0,200 g,

mountain pine oil 0.250 g,

citronella oil 0.050 g,

purified water 42.023 – 42.913 g.

How to take, the dosage

Dolobene should be applied superficially to the skin over the affected area of the skin 2-4 times a day.

When using the gel under the dressing, apply the gel and wait a few minutes to allow it to be absorbed by the skin and evaporate the isopropanol. Then a sealant dressing can be applied.

Dolobene can be used in ionophoresis. The drug is applied under the cathode. Dolobene as a contact gel can be used in ultrasound therapy (phonophoresis). Active substances of the gel complement the therapeutic effect of ultrasound waves.

The duration of treatment depends on the severity of symptoms and the severity of the disease.

Interaction

Applying another drug at the same time with the drug Dolobene may increase their permeability through the skin.

The concomitant use of Dolobene and the non-steroidal anti-inflammatory drug sulindac may cause peripheral neuropathy.

Special Instructions

The place of application of the gel should be free from other medicines, cosmetics or any chemicals. Dolobene must not be applied to the mucous membranes of the eyes, nose, mouth, open wounds or damaged skin (due to radiation, severe sunburn, postoperative scars).

When treating with the drug photosensitivity of the skin may increase, therefore during its use it is necessary to limit intensive sun bathing and visiting a solarium. In case of skin reactions the treatment should be stopped. Due to the high absorption of dimethyl sulfoxide Dolobene should not be used in combination with other ointments and gels.

Contraindications

– hypersensitivity to one of the components of Dolobene gel (active/supplementary substances),
– bronchial asthma,
– severe liver and kidney function disorders,
– expressed disorders of the cardiovascular system,
– pregnancy and lactation,
– age under 5 years,
– open wounds at the application site.

Side effects

There may be local skin reactions (redness, itching, burning sensation at the place of application of the gel), which usually disappear during further treatment. Due to the content of DMSO or other components of the drug, sometimes a skin allergic reaction may occur.

In rare cases, some patients may develop garlic breath odor while using the drug. The smell is caused by dimethyl sulfide, which is a product of dimethyl sulfoxide metabolism. There may be a change in taste sensation, disappearing a few minutes after application of the gel.

In single cases there may be allergic reactions of immediate type (urticaria, Quincke’s edema). Applying the drug on large areas of the body may cause nausea, diarrhea, difficulty in breathing, headache and chills very rarely.

Overdose

There are no data on overdose of the drug.

Pregnancy use

The use of Dolobene during pregnancy and lactation:

Pregnancy:

Because there is no research data on the use of the drug in pregnant women, its use during pregnancy is contraindicated.

The use during lactation:

Dimethylsulfoxide penetrates into breast milk, so breastfeeding should be stopped during use of the drug.
