Selective class II calcium channel blocker, dihydropyridine derivative. Inhibits influx of calcium into cardiomyocytes and vascular smooth muscle cells.
It has antianginal and hypotensive effects. It reduces the tone of the vascular smooth muscle. Dilates coronary and peripheral arteries, reduces myocardial pressure, BP and slightly myocardial contractility, reduces afterload and myocardial oxygen demand. It improves coronary blood flow.
It has practically no antiarrhythmic activity. Does not inhibit myocardial conduction.
Pharmacokinetics
In oral administration, it is quickly absorbed from the gastrointestinal tract. It is subjected to metabolism during “first passage” through the liver. Protein binding is 92-98%.
It is metabolized in liver with formation of inactive metabolites. T1/2 is about 2 hours.
Extracted mainly by kidneys as metabolites and in trace amounts unchanged; 20% is taken out through intestine as metabolites.
Indications
angina pectoris
arterial hypertension.

Excreted primarily by the kidneys in the form of metabolites and in trace amounts unchanged; 20% is excreted through the intestines in the form of metabolites.
Special instructions
To speed up the effect, Cordipin-retard can be chewed. If chest pain occurs during treatment, the drug should be discontinued.
Driving is not recommended at the beginning of treatment. Drinking alcohol leads to an excessive decrease in blood pressure.
It is not recommended to combine the drug Cordipin-retard with nitrates. The combination with beta blockers is often used with success, but requires adequate selection of drug doses.

Contraindications
decrease in blood pressure below 90 mm Hg.
severe heart failure
hypersensitivity to the drug Cordipin-retard.
Side Effects
redness of the skin of the face and neck
headache, nausea, weakness
swelling, palpitations
constipation, abdominal pain
lowering blood pressure
increased angina attacks, deterioration of liver function.
Interaction
When prescribing nifedipine and other antihypertensive drugs, beta-blockers, diuretics, nitroglycerin and long-acting isosorbide simultaneously, the synergistic effect of these drugs (a significant decrease in blood pressure is possible) should be taken into account.
Concomitant administration of fentanyl may further reduce blood pressure. Treatment with nifedipine should be discontinued 36 hours before fentanyl-based anesthesia.
Concomitant use with cimetidine, tricyclic antidepressants, ranitidine may potentiate the antihypertensive effect of nifedipine. Rifampin induces the activity of liver enzymes, accelerating the metabolism of nifedipine, which leads to a decrease in the effectiveness of the latter.
Co-administration of nifedipine may increase the serum concentrations of digoxin, carbamazepine, phenytoin, theophylline and reduce the concentration of quinidine.
With simultaneous use of nifedipine and calcium supplements, the effect of nifedipine is reduced.
It is not recommended to drink grapefruit juice during therapy with Cordipine, due to the possible enhancement of the effects of the drug.
Overdose
Symptoms: marked decrease in blood pressure, acceleration or deceleration of heart rate, irregular heart rhythm, nausea, vomiting, weakness, redness of the skin, dizziness, increased drowsiness, lethargy, convulsions, loss of consciousness.
Treatment: induce vomiting and give activated charcoal. Treatment is symptomatic.
