Selective class II calcium channel blocker, dihydropyridine derivative. Inhibits influx of calcium into cardiomyocytes and vascular smooth muscle cells.
It has antianginal and hypotensive effects. It reduces the tone of the vascular smooth muscle. Dilates coronary and peripheral arteries, reduces myocardial pressure, BP and slightly myocardial contractility, reduces afterload and myocardial oxygen demand. It improves coronary blood flow.
It has practically no antiarrhythmic activity. Does not inhibit myocardial conduction.
Pharmacokinetics
In oral administration, it is quickly absorbed from the gastrointestinal tract. It is subjected to metabolism during “first passage” through the liver. Protein binding is 92-98%.
It is metabolized in liver with formation of inactive metabolites. T1/2 is about 2 hours.
Extracted mainly by kidneys as metabolites and in trace amounts unchanged; 20% is taken out through intestine as metabolites.

How to take, the dosage
For oral administration, the initial dose of Cordypine-retard is 10 mg 3-4 times a day. If necessary, the dose is gradually increased to 20 mg 3-4 times a day.
In special cases (severe arterial hypertension) for a short time the dose may be increased to 30 mg 3-4 times a day.
The maximum daily dose is 120 mg. To treat a hypertensive crisis as well as an attack of angina, Cordypine-retard may be administered under the tongue in 10-20 mg (rarely 30 mg), then the drug will work in 15-20 minutes.
In long-term therapy, retard tablets (long-acting) of 20 mg (rarely 40 mg) 2 times a day are recommended.
Interaction
Concomitant administration of nifedipine with other antihypertensive drugs, beta adrenoblockers, diuretics, nitroglycerin and isosorbide with prolonged action should take into account the synergistic effect of these drugs (significant reduction of blood pressure is possible).
The concomitant administration of fentanyl may additionally decrease blood pressure. Treatment with nifedipine should be discontinued 36 hours prior to anesthesia with fentanyl.
Simultaneous use with cimetidine, tricyclic antidepressants, ranitidine may potentiate the antihypertensive effect of nifedipine. Rifampicin induces the activity of liver enzymes, accelerating the metabolism of nifedipine, which leads to a decrease in the effectiveness of the latter.
The co-administration of nifedipine may increase serum concentrations of digoxin, carbamazepine, phenytoin, theophylline and decrease the concentration of quinidine.
Concomitant use of nifedipine and calcium preparations decreases the effect of nifedipine.
Consumption of grapefruit juice during therapy with Cordypine is not recommended because of the possible increase in the effects of the drug.
Special Instructions
The drug Cordypine-retard may be chewed to speed up the effect. If during treatment pain in the sternum occurs, the drug should be discontinued.
Driving is not recommended at the beginning of treatment. Alcohol consumption leads to an excessive decrease in blood pressure.
The combination of Cordypine-retard with nitrates is not recommended. Combination with beta-blockers, on the other hand, is used frequently and with success, but requires adequate selection of the dosage of the drugs.

Overdose
Symptoms: marked decrease in blood pressure, accelerated or slowed heart rate, irregular heart rhythm, nausea, vomiting, weakness, flushed skin, dizziness, increased drowsiness, lethargy, convulsions, loss of consciousness.
Treatment: induce vomiting and give activated charcoal. Treatment is symptomatic.
