Clodronic acid belongs to the group of bisphosphonates and is an analogue of natural pyrophosphate. The action of bisphosphonates is based on a high affinity for the mineral components of bone tissue. The main mechanism of action of clodronic acid is inhibition of osteoclast activity and reduction of bone tissue resorption mediated by them.
Indications
– osteolytic metastases of malignant tumors in the bone and myeloma (multiple myeloma);
– prevention of the development of bone metastases of primary breast cancer;
– hypercalcemia caused by malignant tumors.
Pharmacological effect

The main mechanism of action of clodronic acid is to suppress the activity of osteoclasts and reduce their mediated bone resorption.
Special instructions
Before and during treatment, the patient should ensure sufficient fluid intake, as well as monitor renal function and calcium concentration in the blood serum.

Contraindications
– pregnancy and breastfeeding;
– concomitant therapy with other bisphosphonates;
– children’s age (due to lack of clinical experience);
– hypersensitivity to clodronic acid, other bisphosphonates or any other components included in the drug Klobir.
Side Effects
Most often, in approximately 10% of patients, adverse reactions such as nausea, vomiting, diarrhea are observed; these reactions usually manifest themselves in a mild form and occur most often when using the drug in high doses.
Interaction
There is evidence of an association between the use of clodronic acid and impaired renal function with the simultaneous administration of non-steroidal anti-inflammatory drugs, most often diclofenac.
Overdose
Symptoms: hypercreatininemia, impaired renal function.
Clinical pharmacology
Absorption of clodronic acid in the gastrointestinal tract occurs quickly and is approximately 2%. Cmax in blood serum after oral administration of one dose of the drug is achieved within 30 minutes. Due to the pronounced affinity of clodronic acid for calcium and other divalent cations, the absorption of clodronic acid is significantly reduced when taking the drug with food or medications containing divalent cations.
